We, Helen Louise Fitch and Mildred Mangham, the undersigned, do hereby
attest that we are the descendants, sole survivors and heirs of Joyce Lewis, our sister,
who is buried in Block 8, Lot 63, Space 1. Joyce Lewis was the owner of Block 8, Lot
63, Spaces 1, 2, 3 and 4 in the Clearwater Municipal Cemetery. We are the executors
of the Estate of Joyce Lewis when she passed away. Spaces 2, 3 and 4 remain empty.
We authorize the granting of interment rights as follows:
Block 8, Lot 63, Space 2: Helen Louise Fitch, sister of Joyce Lewis.
Block 8, Lot 63, Space 3: Kimberly Christy, daughter of Helen Louise Fitch.
Block 8, Lot 63, Space 4: Michael Fitch, son of Helen L. Fitch
Mildred Mangham, other sister of Joyce Lewis, is not interested in these spaces as she
already has other arrangements.
Please take this affidavit as your authority to assign interments rights in spaces
2, 3 and 4 of Block 8, Lot 63.
We agree to release, indemnify and hold the City of Clearwater harmless from
liability in the event of any claim arising from the use of the above described property as
stated above.
