AFFIDAVIT
I, Jean B. Thornton, the undersigned, do hereby attest that I am the sole
survivor and heir of James R. and Susanne E. Blount who are buried in Block 1, Lot 14,
Spaces Nine and Ten, Clearwater Cemetery. I am the heir to the remaining cemetery
spaces which are described as Spaces 1, 2, 3, 4, and 12, Lot 14, Block 1.
Please take this affidavit as your authority to assign interments rights in Space 1,
Lot 14, Block 1 to me, Jean Blount Thornton; Space 2 to my husband, Max C.
Thornton; Space 3, Lot 14, Block 1 to my daughter-in-law, Daryl Detmer Thornton;
Space 4, Lot 14, Block 1 to my son, Grant Blount Thornton, and Space 12, Lot 14,
Block 1 to my son, Max Carl Thornton, Jr.. This document rescinds my previous
request that Max Carl Thornton be granted interment rights to Space 3, Lot 14, Block
1.
I agree to release, indemnify and hold the City of Clearwater harmless from
liability in the event of any claim arising from the use of the above described property as
stated above.
