WE, Theresa J. Gaede, Virginia Concannon, and Vincent Nurfrio, the undersigned, do hereby attest that,
we are the descendants, sole survivors and heirs along with our brother Anthony Nufrio, Sr. (who will sign a separate
affidavit) of FRANK &' KATHERINE NUFRIO, deceased, who are buried in Block Twelve (12), Lot Forty-five
(45), Space(s) Two (2) and Three (3) respectively, in Clearwater Cemetery and who own Lots Two 2, Three 3, Four 4,
and Five 5 in Lot Forty-five (45), Block Twelve (12). We authorize the the granting of interment rights as follows:
Block Twelve (12), Lot Forty-five (45), Space Four (4): Marlene Metzler, grand
daughter of Frank & Katherine Nufrio, and her daughter, Suzanne Metzler (great grand
daughter of Frank & Katherine Nufrio), at least one of which must be an urn.
Block Twelve (12), Lot Forty-five (45), Space Two (2): William A. Proctor, fiance of
Theresa 1. Gaede, daughter of Frank & Katherine Nufrio, in an urn to be placed at the foot of
the space.
Block Twelve (12), Lot Forty-five (45), Space Three (3): Theresa J. Gaede, daughter of
Frank & Katherine Nufrio, in an urn to be placed at the foot of the space.
WE further agree to release, indemnify and hold the City of Clearwater hannless from liability in the event of
any claim arising from the use of the above-described property for the interments as indicated above.
