Block 12, Lot 22, Space 2 in the Clearwater Municipal Cemetery.
Please take this affidavit as your authority to inter the remains of Pasquale V
Paoletti who is the brother of Virginia M Morelli, in Block 12, Lot 22, Space 2.
1, further agree to release, indemnify and hold the City of Clearwater harmless
from liability iri the event of any claim arising from the use of the above described
property for the interment of Pasquale V Paolettl.
