Lot 45, Spaces 2 & 3 (respectively), Clearwater Cemetery. I am the heir to
the cemetery property described as Black 12, Lot 45, Spaces 2, 3, 4 and 5
which are in the name of F~IK NUFRIO, de~~ased, who was my father.
Please
take this affidavit as your authority to grant MARY AGLIATO, 1nY sister, and
VINCENT AGLIATO,', her husband, right of interment in Space Five (5), Lot 45,
Block 12.
I, further agree to release, indemnify and hold the City of Clearwater
harmless from liability in the event of any claim arising from the use or
disposition of the above described property.
