Development Agreement Exhibit ��B" Description of Request: Founded in 1916, Morton Plant Hospital("Morton Plant") was the first hospital built in northern Pinellas County. In an effort to remain one of the Thomson Reuters 100 Top U.S. Hospitals and continue to provide excellent service to the community, Morton Plant is embarking on an expansion effort that will include a new patient tower, an additional main entrance and the relocations of surgery, women's care and orthopedics to better meet the needs of the future. This proposed development agreement will set the framework for Morton Plant's regulatory approvals with the City of Clearwater. A preview master plan was approved in 2002 by the City of Clearwater as a Comprehensive Infill Development approval which will be replaced and superseded upon approval of the proposed Development Agreement. The proposed Master Plan includes construction that will accommodate new parking areas, patient service areas, medical office buildings, outpatient and support services. A more detailed description of the improvements is atta.ched hereto. Submittal Requirements per Code section 4-606.B. 1. A sta.tement of the requested duration of the development agreement, which sha11 not exceed 20 years. Section 5.3 of the proposed Development Agreement states that the duration of the agreement shall be twenty (20) years. 2. A description of all existing and proposed public facilities and services that serve or will serve the development. Section 7 of the Development Agreement includes a statement of public facilities servfng the development. 3. A description of the uses desired to be permitted on the land, including population densities and building intensities and heights. See attached application and site data tables for specifrc information. 4. Identification of zoning district changes, code amendments and comprehensive plan amendments that will be required if the proposed development proposal were to be approved. The applicant does not request any zoning district change, code amendment, or comprehensive plan amendment associated with this Development Agreement. The existing zoning and land use on most of the property is Institutionad; where there are other zoning and Zand use categories, the proposed development is consistent with those categories. Within the development Agreement, the applicant does request confirmation of the legality of the existang nonconforming development with regard to setbacks and height. 5. To accomplish this, the southeast corner of the ED area will be renovated and expanded. The existing canopy will be removed/demolished and the temporary drive demolished. The addition will be constructed with cast-in-place reinforced concrete, structural steel with an exterior skin of stucco and glass to match the adjacent building. The new canopy will be steel construction with translucent panels. A new drive pattern will be created at the temporary ED entry expansion to allow for patient drop-off and valet. o The existing main mechanical and electrical utilities serving the hospital and its campus initiate at the Central Utility Plant (CUP) and feed across the campus. These existing underground utilities will conflict with the proposed new tower foundations and will be relocated. New electrical feeders will be installed between the Central Utility Plant (CUP) and various hospital buildings on campus. New emergency generators and switchgear will be installed in the existing CUP to serve the growing hospital's essential power needs. To serve these new generators, new above-ground diesel fuel tanks will be installed in the parking lot directly across from the CUP. o Demolition of a maintenance building and a portion of a hospital building built in 1941 will be necessary to accommodate the proposed new tower. The hospital will relocate the personnel from those buildings to existing space elsewhere on campus prior to demolition. • Patient Tower Expansion / Renovation Project: o This package is currently in design and documentation phase. o This hospital expansion will be constructed immediately adjacent to the existing hospital buildings and will connect to existing Adler, Morgan, Emergency and current Day-Surgery buildings. Modifications to each of these buildings will be made to accommodate the new construction and tie-ins into the existing hospital buildings. The Adler building will also be expanded on the Ground, 1St and 2"d floors to accommodate new functions. The existing Adler building will have extensive renovations that relate to the proposed functions on the same levels of the new tower. o The primary functions proposed in this project include: ■ Basement of Morgan will be renovated for Sterile Processing and Distribution as well as the relocation Central Sterile Supply. ■ Ground Floor Adler will be renovated for relocated Environmental Services and building mechanical/electrical needs. ■ First Floor will have a new hospital entrance facing east towards the community's approach to the campus. The vehicular canopy construction will match the canopy at the relocated Emergency walk-in entrance. Easy connections and access to Cafeteria, Radiology, Emergency and Patient Rooms will be accomplished too. ■ Second Floor will accommodate relocated and consolidated surgery functions such as Operating Rooms, Prep and Recovery. ■ Third Floor will be the Women's Services floor with Labor & Delivery, C-Section, Post-Partum and NICU. Additional recovery beds will also be provided to support existing Morgan Heart operating rooms. ■ Fourth Floor will be the in-patient Orthopedics floor with new patient rooms and rehab gym. ■ Fifth Floor Adler will be renovated for the Adult Behavioral Unit. o Construction of the proposed new tower will include: Structure: Foundations will be both deep and shallow reinforced concrete meeting the requirements of the Geotechnical analysis and recommendations. Structural frame will be primarily cast-in- place reinforced concrete with some appropriately �re-proofed structural steel. Exterior Enclosure: The exterior design of the new tower and expansions will respect the existing context of the hospital campus buildings through the use of cement plaster (stucco) and non- operable windows. The window system will be an aluminum curtainwall system and storefront window systems that meet the appropriate code required product approvals. The glazing will be a combination of tinted insulated glass for vision, spandrel and translucent lighting as appropriate for views. Exterior insulated �nish system (EIFS) may be used for exterior soffits or otherwise as appropriate. Metal panels or metal panel systems may also be used for enclosure or architectural features. Metal louvers that meet the appropriate code required product approvals will be provided for mechanical air intake or exhaust. As shown, this will require the demolition of the existing Lykes Building. It is anticipated that this new building will connect to the existing hospital at one or more levels. o Future building / parking on the south side of Pinellas Street south of the hospital. This building may include conditioned space for the hospital, structured parking, or both. Morton Plant Hospitat may use this building for leased medical offices or may utilize all, or a portion thereof, for hospital functions. A walkway or bridge may be constructed connecting this future structure to the hospital. o Future building / parking east of the recently completed Axelrod Building on the northwest corner of South Ft. Harrison and Pinellas Street. As shown, this will require the demolition of the existing 1240 Building. This building may include conditioned space for the hospital, structured parking or both. Morton Plant may use this building for leased medical offices or may utilize all, or a portion thereof, for hospital functions. o Future expansion to the existing Rehab Hospital northwest of the comer of South Ft. Harrison and Corbett Street. (!b{'�} 1df RIC�EF RD RU RlOG DEVELOPMENT AGREEMENT THIS DEVELOPMENT AGREEMENT ("Agreement") is dated 2013, effective as provided in Section 5 of this Agreement, and entered into between MORTON PLANT HOSPITAL ASSOCIATION, INC., a Florida not-for-profit corporation ("Morton PIanY'), and THE CITY OF CLEARWATER, FLORIDA, a political subdivision of the State of Florida acting through its City Council, the governing body thereof ("City"). RECITALS: WHEREAS, Sections 163.3220 - 163.3243, Florida Statutes, which set forth the Florida Loca1 Government Development Agreement Act ("Act"), authorize the City to enter into binding development agreements with persons having a legal or equitable interest in real property located within the corporate limits of the City; and WHEREAS, under Section 163.3223 of the Act, the City has adopted Section 4- 606 of the City of Clearwater Community Development Code ("Code"), establishing procedures and requirements to consider and enter into development agreements; and WHEREAS, Morton Plant owns approximately sixty (60) acres of real property located generally at 300 Pinellas Street, Clearwater, FL in the corporate limits of the City ("Property"), which is comprised of several parcels collectively described on Exhibit "A" attached hereto and incorporated herein; and WHEREAS, certain properties incorporated into the Master Site Plan are located within the Town of Belleair, which properties are more specifically described in Exhibit "A-1" attached hereto (the "Belleair Property"); and WHEREAS, the City issued to Morton Plant a Development Order for the Property on January 27, 2003, Case FLD 2002-10035 (the "Previous Development Order"); and WHEREAS, the Previous Development Order shall be replaced and superseded by the terms and conditions of this Agreement; and WHEREAS, Morton Plant has adopted, and is proposing to the City for regulatory approval, a new twenty (20) year Master Site Plan for the redevelopment of the Property (the "Master Site Plan"); and WHEREAS, Morton Plant is seeking regulatory approval from the City for the development and redevelopment of the Property in accordance with the Master Site Plan, as more specifically described hereinafter; and WHEREAS, the City has conducted such hearings as required by and in accordance with Chapter 163.3220 Fla. Stat. (2008) and any other applicable law; and WHEREAS, the City has determined that, as of the Effective Date of this Agreement, the Master Site Plan is consistent with the City's Comprehensive Plan and Code; and WHEREAS, the City has conducted public hearings as required by Sections 4-206 and 4-606 of the Code; and WHEREAS, the Community Development Board ("CDB") recommended approval of the Master Site Plan on , 2013, conditioned upon the approval and execution of this Agreement; and WHEREAS, at a duly called and advertised public meeting on , 2013, the City Council approved this Agreement and authorized and directed its execution by the appropriate officials of the City; and • WHEREAS, Morton Plant has approved this Agreement, and has duly authorized certain individuals to execute this Agreement on Morton Plant's behalf. STATEMENT OF AGREEMENT In consideration of and in reliance upon the premises, the mutual covenants contained herein, and other good and valuable consideration, the receipt and su�ciency of which are hereby acknowledged, the parties hereto intending to be legally bound and in accordance with the Act, agree as follows: SECTION 1. Recitals. The above recitals are true and correct and are a part of this Agreement. SECTION 2. Incornoration of the Act. This Agreement is entered into in compliance with and under the authority of the Code and the Act, the terms of which as of the date of this Agreement are incorporated herein by this reference and made a part of this Agreement. Words used in this Agreement without definition that are defined in the Act shall have the same meaning in this Agreement as in the Act. SECTION 3. Pronertv Subiect to this A�reement. The Property is subject to this Agreement. SECTION 4. Scoae of Proiect. 2 4.1 The project sha11 be in substantial conformance with the twenty (20) year Master Site Plan dated May 2013 and prepared by GS&P, Architects, a copy of which is attached hereto as Exhibit "B" ("Master Site Plan"). A larger and more detailed copy of the Master Site Plan shall be maintained for inspection and review at the City Planning Department, and the Office of the City Clerk. 4.2 The Master Site Plan shall have no more than six hundred eighty seven (687) inpatient hospital beds, as reflected on the Master Site Plan. 4.3 The Master Site Plan shall have no more than Five Hundred Six Thousand Seven Hundred Forty Nine (506,749) square feet of space utilized as medical offices, as more specifically depicted on the Master Site Plan. 4.4 The Master Site Plan sha11 have no more than Seventy-five Thousand (75,000) square feet of space utilized for the purpose of ambulatory outpatient services, as more specifically depicted on the Master Site Plan. 4.5 The Master Site Plan shall include normal and customary accessory and ancillary uses necessary to the operation of an inpatient hospital including but not limited the morgue, hospital administration offices, cafeteria, surgical floors, laundry facilities, waiting areas, gift stores and education classrooms. 4.6 The Master Site Plan sha11 include no less than Four Thousand Eight Hundred Seventy Eight (4,878) parking spaces, as depicted on that certain Parking Zone Plan, dated May 2013 by GS& P, Architects, copy of which is attached hereto as Exhibit "C" and hereby incorporated by reference (the "Parking Zone Plan"). A larger and more detailed copy of the Parking Zone Plan shall be maintained for inspection and review at the office of the City Planning Department, and the Office of the City Clerk. Morton Plant shall be permitted to and sha11 maintain sufficient parking ratios to support the Master Plan improvements based upon the actual square foot constructed per use identified in sections 4.2, 4.3 and 4.4 herein and minimum Code requirements. This obligation shall not supersede the interim parking requirements as discussed in section 4.11 below. 4.7 The landscape buffer and property setback requirements for the Property sha11 be as more depicted on Exhibit "D" attached hereto and incorporated herein (the "Buffer and Setback Plan"). A larger and more detailed copy of the Buffer and Setback Plan shall be available for inspection and review at the office of the City Planning Department, and the Office of the City Clerk. As a deviation to the provisions of the Code, the setback and landscape buffer requirements shall not be applied on subparcels of the Property which are bisected by public right-of-way, and where all adjacent and contiguous subparcels are owned by Morton Plant, as such areas are specifically depicted on Exhibit "E" attached hereto and hereby incorporated by reference (the "Exempt Areas"). 3 4.8 Ratifications of Existing Deviations. Motion Plant and the City acknowledge that certain existing structures located on the Property are not in compliance with the provisions of the Code. All such known deviations are set forth in Exhibit "F" attached hereto and hereby incorporated by reference. The City agrees that a11 such existing non- conformities are hereby granted a deviation from compliance from the Code to reflect the existing circumstance 4.9 The height, as that term is defined in the Code, for any structure located on the Property, sha11 be one hundred fifty (150) feet, provided, however, that: within 100 feet of the campus edge, as defined in the Buffer and Setback Plan, no structure may exceed 92 feet in height. 4.10 Prior to the expiration of this Agreement, Morton Plant agrees to record a Unity of Title encumbering a11 of the Property, in a form reasonably acceptable to the City Attorney and Morton Plant. 4.11 The City acknowledges and agrees that the improvements contemplated under the Master Site Plan will be constructed in phases. Accordingly, certain subparcels located with the Property that are shawn for future types of development may, on occasion, be used for interim pazking and staging purposes, to facilitate the effectuation of a11 of the improvements contemplated by the Master Site Plan. Accordingly, Morton Plant shall have the right to use the Property for interim sta.ging and parking, provided that the interim parking and sta.ging complies with the dimensional restrictions (including but not limited to floor area ratio, impenetrable surface ratio and lot size) identified in this Master Site Plan and the Code. 4.12 The City acknowledges and agrees that there is an important public puxpose served by permitting each structure identified on the Master Plan to have its own signage, and that wayfinding and directional signage is of great public importance on the Property and therefore Morton Plant shall utilize the Comprehensive Sign Program to provide sufficient signage on each structure included within the Master Plan. 4.13 Morton Plant sha11 provide a transit stop, the size and specific location as mutually agreed upon by the City and Morton Plant, to the Property in the area generally shown on Exhibit G attached hereto and made part hereof. SECTION 5. Effective DatelDuration of this A�reement. 5.1 This Agreement shall not be effective until 5.1.1 this Agreement is properly recorded in the public records of Pinellas County, Florida, and thirty (30) days have elapsed after having been received by the Department of Community Affairs pursuant to Florida Sta.tutes Section 163.3239 and Code Section 4-606.G.2; 4 5.2 Within fourteen (14) days after the City approves the execution of this Agreement, the City shall record the Agreement with the Clerk of the Circuit Court for Pinellas County. Morton Plant shall pay the cost of such recording. The City sha11 submit to the Department of Community Affairs a copy of the recorded Agreement within fourteen (14) days after the Agreement is recorded. 5.3 This Agreement shall continue in effect until terminated, as defined herein, for a period of twenty (24) years. SECTION 6. Obli�ations under this Agreement. 6.1 Obli�ations of Morton Plant 6.1.1 The obligations under this Agreement shall be binding upon, and the benefits of this Agreement shall inure to Morton Plant, its successors in interests or assigns. 6.1.2 At the time of development of the improvements described in the Master Site Plan, Morton Plant sha11 submit such applications and documentation as required by law and shall comply with the City's building code applicable at the time of building review, provided, however, if such application is generally consistent with the Master Site Plan and the provisions of this Agreement, no additional pubic hearings sha11 be required. 6.1.3 The following restrictions shall apply to development of the Property: 6.1.3.1 T'he Property and improvements located thereon shall be developed consistent with the Master Site Plan. Any minor revisions or changes to the Master Site Plan shall be submitted to and reviewed by the Planning and Development Director as a minor modification, pursuant to the Code. Any modifications determined by the Planning and Development Director as either inconsistent or constituting a substantial deviation from the approved Master Site Plan and thus requiring further approval by the CDB shall require an amendment to this Agreement in accordance with the procedures of the Act and the Code, as necessary and applicable, provided, however, that the rights created herein shall remain undisturbed Any and a11 such approved and adopted amendments sha11 be recorded in the public records of Pinellas County, Florida. 6.1.3.2 Morton Plant shall obtain building permits and shall thereafter timely obtain required certificates of occupancy in accordance with the approved development order for the Master Site Plan, and Code Section 4-407. Nothing herein shall restrict Morton Plant from seeking an extension of these time frames pursuant to applicable provisions of the Code and of the Florida Building Code or from seeking an amendment to this Agreement. 5 6.2 Obligations of the Citv. 6.2.1 The City sha11 promptly process site and construction plan applications for the Property that axe consistent with the Comprehensive Plan and this Agreement, and that meet the requirements of the Code. 6.2.2 The final effectiveness of the re-designations referenced in Section 6.2.1 is subject to: 6.2.3.1 T'he provisions of Chapters 163 and 166, Florida Statutes, as they may govern such amendments; and 6.23.2 The expiration of any appeal periods or, if an appeal is filed, the conclusion of such appeal. SECTION 7. Public Facilities to Service Develonment. The following public facilities are presently available to the Property from the sources indicated below. Development of the Property will be governed by the concurrency ordinance provisions applicable at the time of development approval, unless otherwise provided by law. With respect to transporta.tion and other public infrastructure and services subject to concurrency requirements, all applicable concurrency provisions for the proposed development have been met. 7.1 Potable water is available from the City. Morton Plant shall be responsible for all necessary main extensions and applicable connection fees. 7.2 Sewer service is currently provided by the City. Morton Plant shall be responsible for all necessary main extensions and applicable connection fees. 73 Fire protection from the City. 7.4 Drainage facilities for the Property will be provided by Morton Plant at Morton Plant's sole expense. 7.5 Transportation concurrency requirements have been met. SECTION 8. Required Local Government Permits. The required local government development permits for development of the Property include, without limitation, the following: 8.1 Construction plan approval(s); 8.2 Utility licenses, access, and right-of-way utilization permits; 8.3 Building permit(s); and :� 8.4 Certificate(s) of occupancy. SECTION 9. Consistencv. T'he CiTy finds that as of the Effective Date of this Agreement, the Master Site Plan is consistent with the City Comprehensive Plan and the Code. SECTION 10. Termination. 10.1 If Morton Plant's obligations set forth in this Agreement are not followed in a timely manner, as reasonably determined by the City Manager, after notice to Morton Plant and an opportunity to be heard, existing permits sha11 be administratively suspended and issuance of new permits suspended until Morton Plant has fulfilled its obligations. Failure to timely fulfill its obligations may serve as a basis for termination of this Agreement by the City, (in addition to the termination provisions of Section 5.2 hereo fl, at the discretion of the City and after a minimum thirty (30) day notice to Morton Plant, an opportunity for Morton Plant to cure the default, and an opportunity for Morton Plant to be heard. SECTION 11. Other Terms and Conditions. 11.1 Except in the case of termination, until twenty (20) years after the date of this Agreement, the Property shall not be subject to down-zoning, unit density reduction, or intensity reduction, unless the City has held a public heazing and determined: 11.1.1 That substantial changes have occurred in pertinent conditions existing at the time of approval of this Agreement; or 11.1.2 This Agreement is based on substantially inaccurate information provided by Morton Plant; or 11.1.3 That the change is essential to the public health, safety, or welfaxe. SECTION 12. Comaliance with Law. The failure of this Agreexnent to address any particular permit, condition, term or restriction shall not relieve Morton Plant from the necessity of complying with the law governing such permitting requirements, conditions, terms or restrictions. SECTION 13. Notices. ASSIGNMENTS. 14.1 By Morton Plant. Morton Plant may sell, convey, assign or otherwise dispose of any or all of its right, title, interest and obligations in the Property, in its entirety, without the prior written notice to the City, provided that such party (hereinafter referred to as the "assignee"), to the extent the sale, conveyance, assignment or other disposition by Morton Plant to the assignee, shall be bound by the terms of this Agreement the same as Morton Plant for such part of the Property or improvements as is subject to such sale, conveyance, assignment or other disposition, and Morton Plant shall be released from such obligations that have been assumed by the by the Assignee. 14.2 Successors and Assigns. The terms herein contained shall bind and inure to the benefit of the City, and its successors and assigns, and Morton Plant and, as applicable to the parties comprising Morton Plant, their successors and assigns, except as may otherwise be specifically provided herein. SECTION 15. Minor Non-Compliance. Morton Plant will not be deemed to have failed to comply with the terms of this Agreement in the event such non- compliance, in the judgment of the City Manager, reasonably exercised, is of a minor or inconsequential nature. SECTION 16. Covenant of Cooneration. The parties sha11 cooperate with and deal with each other in good faith and assist each other in the performance of the E:j provisions of this Agreement and in achieving the completion of development of the Property. SECTION 17. Annrovals. Whenever an approval or consent is required under or contemplated by this Agreement such approval or consent shall not be unreasonably withheld, delayed or conditioned. All such approvals and consents sha11 be requested and granted in writing. SECTION 18. Completion of A�reement. Upon the completion of performance of this Agreement or its revocation or termination, a statement evidencing such completion, revocation or termination shall be signed by the parties hereto and recorded in the official records of the City. SECTION 19. Entire A�reement. This Agreement (including any and all Exhibits attached hereto all of which are a part of this Agreement to the same extent as if such Exhibits were set forth in fu11 in the body of this Agreement), constitutes the entire agreement between the parties hereto pertaining to the subject matter hereof. SECTION 20. Construction. The titles, captions and section numbers in tlus Agreement are inserted for convenient reference only and do not define or limit the scope or intent and should not be used in the interpretation of any section, subsection or provision of this Agreement. Whenever the context requires or permits, the singular shall include the plural, and plural shall include the singular and any reference in this Agreement to Morton Plant includes Morton Plant's successors or assigns. This Agreement was the production of negotiations between representatives for the City and Morton Plant and the language of the Agreement should be given its plain and ordinary meaning and should not be strictly construed against any party hereto based upon draftsmanship. If any term or provision of this Agreement is susceptible to more than one interpretation, one or more of which render it valid and enforceable, and one or more of which would render it invalid or unenforceable, such term or provision shall be construed in a manner that would render it valid and enforceable. SECTION 21. Partial Invaliditv. If any term or provision of this Agreement or the application thereof to any person or circumstance is declaxed invalid or unenforceable, the remainder of this Agreement, including any valid portion of the invalid term or provision and the application of such invalid term or provision to circumstances other than those as to which it is held invalid or unenforceable, shall not be affected thereby and shall with the remainder of this Agreement continue unmodified and in full force and effect. Notwithstanding the foregoing, if such responsibilities of any party hereto are thereby limited, to the extent that the purpose of this Agreement or the benefits sought to be received hereunder are frustrated, such party shall have the right to terminate this Agreement upon fifteen (15) days written notice to the other parties. SECTION 22. Code Amendments. Subsequently adopted ordinances and codes of the City which is of general application not governing the development of land sha11 be applicable to the Property, and such modifications are specifically anticipated in this Agreement. E SECTION 23. Governin� Law. This Agreement sha11 be governed by, and construed in accordance with the laws of the State of Florida without regard to the conflict of laws principles of such sta.te. SECTION 24. Counterparts. This Agreement may be executed in counterparts, all of which together shall continue one and the same instrument. SECTION 25. Amendment. July 2013 a,� �i�{�},����r` i,�,��i����/E 2.1 Introduction and Purpose This report summarizes the findings and work for the overall drainage infrastructure for the Morton Plant Hospital campus, which is in support of the Development Agreement to which it is attached. The campus is located in the City of Clearwater, Florida within section, Township x, Range x. The hospital campus consists of several non- contiguous properties separated by public right-of-ways and is situated on approximately 73 acres. Please see ExhibitA, for an aerial of the campus. `3.�} EXi$TlNG Ct?Nt��TiC1NS 3.1 Current Land Use The campus consists of a fully developed urban hospital campus with associated structures, utility infrastructure, pedestrian facilities and parking (both surface and structured). The majority of the campus is covered under existing SWFWMD ERP permits, which are identified in the ExhibitA. 3.2 Soils Information Based on the "Soils Survey of Pinellas County, Florida", published by the United States Department of Agriculture Soil Conservation Services (SCS), the primary soil types that are located within the project area are: • Astatula Soils (Map Symbol 4) • Myakka Soil (Map Symbol 17) • Matlacha and St. Augustine Soils (Map Symbol 16) 3.3 Drainage As expressed above, the majority of the campus is covered under existing SWFWMD ERP permits. There are a total of four (4) areas currently not covered under an existing SWFWMD ERP permit. Additional SWFWMD ERP permits will be applied for as future development approvals are sought for site construction. 3.4 Wetlands There is one (1) wetland area located within the campus boundary, which is located along the south side of Pinellas Street at the southwest corner of Pinellas Street and Reynolds Street. The proposed master plan that is included in the Development Agreement proposes future building/parking within this area. It is understood the future site will have to undergo wetland impacUmitigation permitting in order to facilitate a permit through the various State, local and Federal agencies. The wetland area conveys significant offsite runoff from the south to a large (72") culvert located along the north side of Pinellas Street, which ultimately outfalls into Clearwater Bay. The future building/parking development will provide for adequate drainage infrastructure to bypass the offsite flows in order to maintain the existing drainage patterns and avoid adverse impacts upstream/downstream of the future development. 3.5 Flood Plain Information Based on information shown on the Federal Emergency Management Agency (FEMA) Federal Insurance Rate Maps (FIRM) map (Panel #:12103C0108H, dated May 17, 2005) the site lies within Flood Zones "VE", "AE", and "X". The one hundred (100) year flood elevation for the portion of the site within Zone "VE" is 13.0 and Zone "AE" Morton Plant Hospital Master Stormwater Management Summary Development Agreement Support Project No. 215610486 Morton Plant Hospital Association, Inc. July 2013 is 12.0. Given that the site falls within a costal flood zone, no mitigation is required for any potential flood plain impacts of future development. Please see Exhibit B, F/ood /nsurance Rate Map (F/RM) for the FIRM map of the campus. 3.6 Existing Water Quality and Treatment Provided Several onsite treatment facilities exist on the campus, which are covered under the existing SWFWMD ERP permits referenced in ExhibitA. As future development occurs a site specific drainage analysis will be performed the each respective development such that it will be brought up to code by providing adequate treatment facilities. These future treatment facilities will be the combination of underground storage facilities and above ground, open- aired facilities that will adhere to both SWFWMD and City of Clearwater criteria. Water quantity is not required for the Morton Plant Hospital campus as it is within a tidal watershed that has direct outfall to Clearwater Bay. However, with each respective development, a site specific analysis will be performed to demonstrate the treatment facility's ability to accommodate not only the required treatment volume, but also the required storm events (25yr/24hr and 100yr/24hr events). Book 12139, Page 579, re- recorded in Book 12386, Page 79 G R E S H A M S M I T H A N D P A R T N E R S June 3, 2013 20-YEAR MASTER SITE PLAN — ARCHITECTURAL NARRATIVE MORTON PLANT HOSPITAL CLEARWATER, FLORIDA GS&P Project No. 28663.00 Proposed Patient Tower —(Proiects Currentiv Underwav) Morton Plant Hospital is currently in the planning and construction stages of campus improvements that include a new patient tower and renovations of existing spaces that will be constructed in multiple construction packages including: • Emergency Entrance Relocation / Utilities / Internal Relocations / Demolition: o This package has been issued, permitted and is currently under construction. o The existing Emergency Department (ED) walk-in entrance to be relocated to allow the construction of the new tower. To accomplish this, the southeast corner of the ED area will be renovated and expanded. The existing canopy will be removed/demolished and the temporary drive demolished. The addition will be constructed with cast-in-place reinforced concrete, structural steel with an exterior skin of stucco and glass to match the adjacent building. The new canopy will be steel construction with translucent panels. A new drive pattern will be created at the temporary ED entry expansion to allow for patient drop-off and valet. o The existing main mechanical and electrical utilities serving the hospital and its campus initiate at the Central Utility Plant (CUP) and feed across the campus. These existing underground utilities will conflict with the proposed new tower foundations and will be relocated. New electrical feeders will be installed between the Central Utility Plant (CUP) and various hospital buildings on campus. New emergency generators and switchgear will be installed in the existing CUP to serve the growing hospital's essential power needs. To serve these new generators, new above-ground diesel fuel tanks will be installed in the parking lot directly across from the CUP. o Demolition of a maintenance building and a portion of a hospital building built in 1941 will be necessary to accommodate the proposed new tower. The hospital will relocate the personnel from those buildings to existing space elsewhere on campus prior to demolition. • Patient Tower Expansion / Renovation Project: o This package is currently in design and documentation phase. o This hospital expansion will be constructed immediately adjacent to the existing hospital buildings and will connect to existing Adler, Morgan, Emergency and current Day-Surgery buildings. Modifications to each of these buildings will be made to accommodate the new construction and tie-ins into the existing hospital buildings. The Adler building will also be expanded on the Ground, 1St and 2"d floors to accommodate new functions. The existing Adler building will have extensive renovations that relate to the proposed functions on the same levels of the new tower. o The primary functions proposed in this project include: ■ Basement of Morgan will be renovated for Sterile Processing and Distribution as well as the relocation Central Sterile Supply. ■ Ground Floor Adler will be renovated for relocated Environmental Services and building mechanical/electrical needs. First Floor will have a new hospital entrance facing east towards the community's approach to the campus. The vehicular canopy construction will match the canapy at the relocated Emergency walk-in entrance. Easy connections and access to Cafeteria, Radiology, Emergency and Patient Rooms will be accomplished too. ■ Second Floor will accommodate relocated and consolidated surgery functions such as Operating Rooms, Prep and Recovery. ■ Third Floor will be the Women's Services floor with Labor & Delivery, C-Section, Post-Partum and NICU. Additional recovery beds will also be provided to support existing Morgan Heart operating rooms. ■ Fourth Floor will be the in-patient Orthopedics floor with new patient rooms and rehab gym. ■ Fifth Floor Adler will be renovated for the Adult Behavioral Unit. o Construction of the proposed new tower will include: Structure: Foundations will be both deep and shallow reinforced concrete meeting the requirements of the Geotechnical analysis and recommendations. Structural frame will be primarily cast-in- place reinforced concrete with some appropriately fire-proofed structural steel. Exterior Enclosure: The exterior design of the new tower and expansions will respect the existing context of the hospital campus buildings through the use of cement plaster (stucco) and non- operable windows. The window system will be an aluminum curtainwall system and storefront window systems that meet the appropriate code required product approvals. The glazing will be a combination of tinted insulated glass for vision, spandrel and translucent lighting as appropriate for views. Exterior insulated finish system (EIFS) may be used for exterior soffits or otherwise as appropriate. Metal panels or metal panel systems may also be used for enclosure or architectural features. Metal louvers that meet the appropriate code required product approvals will be provided for mechanical air intake or exhaust. Future Buildinqs on Campus • As part of Morton Plan Hospital's 20-year master site plan, other future buildings are proposed: o Future building just south of the existing inpatient hospital buildings. As shown, this will require the demolition of the existing Lykes Building. It is anticipated that this new building will connect to the existing hospital at one or more levels. o Future building / parking on the south side of Pinellas Street south of the hospital. This building may include conditioned space for the hospital, structured parking, or both. Morton Plant Hospital may use this building for leased medical offices or may utilize all, or a portion thereof, for hospital functions. A walkway or bridge may be constructed connecting this future structure to the hospital. o Future building / parking east of the recently completed Axelrod Building on the northwest corner of South Ft. Harrison and Pinellas Street. As shown, this will require the demolition of the existing 1240 Building. This building may include conditioned space for the hospital, structured parking or both. Morton Plant may use this building for leased medical offices or may utilize all, or a portion thereof, for hospital functions. o Future expansian to the existing Rehab Hospital northwest of the corner of South Ft. Harrison and Corbett Street. The buildings' structural frame may be either cast-in-place reinforced concrete and/or fire-proofed structural steel construction. Exterior Enclosure: The exterior design of the future buildings and expansions will respect the existing context of the hospital campus buildings through the use of any combination of the following exterior materials including, but not limited to: cement plaster (stucco), precast concrete, metal panel system, exterior insulated finish system, gypsum fiber reinforced concrete panels, stone, wood, brick, or other appropriate material satisfactory to the hospital. The window system that meets the appropriate code required product approvals will be any combination of aluminum curtainwall system and/or storefront window systems that meet the appropriate code required product approvals. Glazing will be any cambination of clear or tinted, single-glazed or insulated, glass for vision, spandrel and translucent glass, all as deemed appropriate for views or aesthetic appeal. Metal louvers that meet the appropriate code required product approvals will be provided for mechanical air intake or exhaust. Exterior entrance and exit doors will be either painted metal or aluminum and glass and may have automatic operators if appropriate.
