Space Agreement you entered into with the City of Clearwater
on September 29, 1999.
We have not heard from you or received any payment from you
since your $50.00 non-refundable deposit on that day.
We must receive the balance of $464.00 in full by September
1, 2000, in order to save that space for you. Please let us
hear from you as soon as possible.

CONTRACT TO PURCHASE CEMETERY SPACE
1
THIS CONTRACT, made and entered into this day of
19 between the City of Clearwater as "G NTOR" and
Willi ;Jean Turner, vhose mailing address is 706 Palm Bluff Street,
Clearwater 33755, of the County of Pinellas, State of Florida, as
"PURCHASER":
WITNESSETH: That in consideration of the mutual promises and
covenants herein contained, and other valuable considerations, the Grantor
agrees to sell and the Purchaser agrees to buy the following described
space: SPACE 1, LOT 321, BLOCK 14, Clearwater Cemetery, as recorded
in Plat Book 60, Page 30, Public Records of Pinellas County, Florida.
PURCHASE PRICE $ 500.00
less Deposit $ 50.00
Balance due $ 450.00 plus $14.00 Recording fee
PAYABLE in six monthly installments, beginning October 29, 1999, with five (5) monthly installments of $75.00 and
one (1) final payment of $89.00 which includes a monthly payment of $75.00 and the recording fee of $14.00.
In the event of default in any monthly installment, the entire sum may be immediately declared due in full, and failure of
the purchaser to make said payment shall entitle the Grantor to retain all proceeds theretofore paid, as liquidated damages, and
may also retain title to any part or portion of said property which has not been used for grave space, and thereupon this
contract shall become null and void.
NO SPACE SHALL BE USED AS A BURIAL SITE UNTIL THE CONTRACT HAS BEEN PAID IN FULL.
THE PURCHASER FURTHER AGREES to abide by all rules and regulations governing the use and maintenance of
space(s) in Clearwater Cemetery. This contract shall not be assigned nor shall any grave space sold hereunder be transferred
without the written consent of the City of Clearwater.
The obligations and benefits under this contract shall extend to the personal representatives and heirs of the Purchaser
and the successors of the City.

The Purchaser will first visit the cemetery and obtain the legal description of the space selected from the caretaker at the cemetery. The
following steps shall then be followed in their listed order.

Input ownehship information in data base program for spaces in all Blocks. Additionally, write information in Ownership
Book (lo ted in vault) for all spaces in Blocks 1 thru 11 only
-Prepare X for each space purchased
Prepare routing memo
Fill in Florida Dept. of Revenue form
-Enter document information re deed in Document Tracking System
Route deed and revenue form for signature -file check list in vault (28-003)
-After deed is returned to Clerk Dept. fully executed, send for recording and enter necessary information in Document
Tracking System
-Upon return of deed from recording, pull check list from 28-003, mark completed in Document Tracking System, and enter
recording date and OR Book & Page number in Cemetery data base file
-Prepare transmittal letter and enclose the original deed and a copy of the Rules and Regulations and send to owner
-File copy of deed, check list, and transmittal letter (attached in that order) in cemeteryfiles in vault by owner's last name in
28-001
ADDITIONAL STEPS FOR AT-NEED USE OF SPACE (INTERMEN7)
Fill out burial permit (located in overhead cabinet at Receptionist's station)
-Fax permit to cemetery (6487) and Parks field house (6551)
-File permit in back of blue binder located in vault above the 28-000 files
-Prepare receipt when staking fee ($30.00) is received from Funeral Home
(mark in data base file and on paper copy of cemeterypermit)
