I am enclosing an affidavit you discussed with my Supervisor, Susan Stephenson, on
December 4, 2000. If you wish to assign interment rights to your son, John Thomas Rooney, you
will need to sign the affadavit where indicated. You will also need one other witness and a Notary
at the time of signing.
The City is unable to transfer ownership of this property; we can only assign interment
rights based on the fact that you are attesting to your relationship to the lot owner of record,
Thomas T. Mc Guire, deceased, by way of affadavit.

Space 4.
WE further agree to release, indemnify and hold the City of Clearwater harmless from liability in the event of
any claim arising from the use of the above-described property for the interment of John Thomas Rooney.
