Yes, they may use this format, but the statutory requirement requires a 3" box in the upper right hand corner be
blank, which is why the other sample showed the margin askew.
Attached to it should be the Consent form but omitting the Mayor's signature.
I will keep the forms in our files for future reference as we did not have them.

The Deed portion is not correctly titled.
The document is not correctly formatted at the top, the text should extend to the right margin.
The Consent to Conveyance was not executed by the Mayor in previous examples, to my recollection.
I believe the City Manager has signed, as I do not recall these going to the City Council.
