It(l) Whenever any clerk or inspector of election,
shall doubt that the handwriting of the signature affixed to
a signature identification slip by•any elector at the time of
presenting himself at the polls to vote is the same as that
of the signature of the named elector affixed in the registra-
tion book at the time of registration, it shall thereupon be-
come the duty of such clerk or inspector of election to forth-
with deliver to the person'so presenting himself to vote a
form of affidavit, which shall be in substantially the follow-
ing-form.
