Be It Ordained by the Town Council of the Town of Clearwater, Florida: Section 1. That the Town Clerk shall be Local Registrar of vital statistics, and it shall be his duty to supply to all physi- cians, midwives, undertakers and others requiriazg them for use, blank certificates for the registration of births and deaths,, which shall be of the standard form approved by the State Board of Health, and to receive and record such certificates in a book. i provided for that purpose, and transmit the originals to the State Board of Health not later than the tenth day of each month. Sec. 2. It shall be the' duty of the Health officer of said Town to issue burial, removal or other permits upon the 4 , receipt of complete, satisfactory and properly executed certificates of death; and no dead body of a human being whose death occurs or. F ' 3 whose body is found in the Town limits of said, Town, or which shall be brought into said Town, shall be interred, or otherwise 1 disposed of, or removed from the Town, or held more than seventy- two hours after death, without a burial, removal or other permit ! issued by the Health Officer in advance of such interment or other. ` disposition or removal. The Health Officer shall file a copy of all such permits issued by him with the Town Clerk of said Torts, :. who shall make a record of the same in the book provided for this , purpose. The person applying for such permit shall pay to the Health Officer a fee of Twenty-five Cents for issuing the same. Sea. 3. That it shall be the duty of the physician, ?. midwife, or person acting as midwife, attending a case of. " confinement in the Tovrn of Clearwater, to file a complete, satisfactory and properly executed certificate of birth with the Town Clerk within three days thereafter; provided, that when there was no physician or midwife in attendance, then it shall be the duty of the parent, householder or ovrner of the premises where the birth occured to file such certificate. Sec. 5. That it shall be the duty of the Health Officer to enforce this Ordinance, and to report to the City Attorney all violations thereof, and it shall be the, duty of the City Attorney upon notice from the Health Officer or any other r. .3 person, of a violation of this Ordinance, to promptly in- vestigate the case and institute necessary proceedings for ..' ;, the imposition of the penalties herein provided. Sec. 6. That any physician, midwife, undertaker, or other person, who violates or fails to comply with any of the provisions or duties of this Ordinance or any Section thereof., 9- shall., upon trial and conviction thereof, be punished by a `:. fine not exceeding Two Hundered Dollars or imprisonment in the Town Jail or at labor upon the streets or other public works of said Town not exceeding ninety days, or by both such fine - and imprisonment, at the discretion of the Mayor. ,' • ; ? : ' ,1 Passed by the Town Council of the Town of Clearwater, Florida, this 4th day of November A.
