RESOLUTION
No. "00 -84
A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF CLEARWATER.
FLORIDA. AUTHORIZING CHANGE OF SIGNATURES ON BANK ACCOUNT OF
CITY OF CLEARWATER SELF INSURANCE PROGRAM.
WHEREAS, the City of Clearwater previously entered into on agreement
with Gollagher Bassett Insurance Service to administer the self insurance
program for the City of Clearwater; and established B separate bsnk account
to handle this program; and
WHEREAS, Gallagher Bassett Insurance Services has had a change in
personnel that requires the changing of the Signatures required on said bank
account;
NOW. THEREFORE. BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF CLEARWATER, FLORIDA, IN SESSION DULY AND REGULARLY
ASSEMBLED. AS FOLLOWS:
1. That per the request of Gallagher Bassett Insurance Services,
the personnel authorized to sign on the City of Clearwater Self Insurance
Program Account at Barnett Bank of Clearwater, N.A., be as follows:
Don D. Swenson
Branch Manager
Jon C. Marcin - Claim Representative
P.J. Curran - Clatm Representative
Lucy McNeill - Claim Analyst
Angela Howard - Branch Secretary
2. .That certified copies of this Resolution be forwarded forthwith to
Barnett Bonk of Clearwater. N.A. and said copy shall be the official
authorization of the City of Clearwater to said Bank to accept the signatures
of Don D. Swenson, Jon C. Marcin. P.J. Curran. Lucy McNeill. or Angela
Howard immediately.
3. That this Resolution shall be effective immediately upon its
adoption.
