A RESOLUTION OF>THE CITY COMMISSION OF THE
CITY OF CLEARWATER, FLORIDA, AU'T'HORIZING
ESTABLISHMENT OF A BANK ACCOUNT FOR THE
PURPOSES OF ADMINISTERING THE CITY OF
CLEARWATER SELF INSURANCE PROGRAM.
WHEREAS, the City of Clearwater has entered into an agreement
with Gallagher Bassett Insurance Service to administer the self insurance
program for the City of Clearwater; and
WHEREAS, such agreement calls for the establishment of a separate
bank account to handle said program;
NOW, THEREFORE, BE IT RESOLVED BY THE CITY
COMMISSION OF THE CITY OF CLEARWATER, FLORIDA,
IN SESSION DULY AND REGULARLY ASSEMBLED, AS
FOLLOWS:
1. That the City of Clearwater, establish a bank account at Barnett
Bank of Clearwater, N. A. entitled "City of Clearwater - Self Insurance
Program, "
2. That as Administrators of this account, any one of the following
officials of Gallagher Bassett Insurance Service be authorized to sign on
said account:
Don D. Swenson - Branch Manager
Linda Stofel - Branch Secretary
Lucy McNeill - Claims Clerk
Jon Marcin - Claims Representative
3. That certified copies of this Resolution be forwarded forthwith to
Barnett Bank of Clearwater, N. A. and said copy shall be the official
authorization of the City of Clearwater to said Bank to accept the signatures
of Don D. Swenson, Linda Stofel, Lucy McNeill, or Jon Marcin immediately. .
4. That this Resolution shall be effective immediately upon its
adoption.
