A RESOLUTION OF THE CIT'i COMMISSION OF TIm CITY OF CLEARWATER,
FLORIDA, AUTHORIZING BARNETT BANK OF PINELLAS COUNTY, N.A. TO
ACCEPT REQUESTS FROH GALLAGHER BASSETT INSURANCE SERVICES FOR
ELECTRONIC TRANSFERS OF FUNDS ON BANK ACCOUNT OF CITY OF
CLEARWATER SELF INSURANCE PROGRAM.
WHEREAS, the City of Clearwater previously enterad into an agreement

with Gallagher Bassett Insurance Service to administer the self insurance program
for the City of Clearwater. and established a separate bank account to handle this

program; and
WHEREAS, Gallagher Bassett Insurance Services has established new
procedures that require that funds be electronically transferred to a central
bank account to pay for the claims of the City of Clearwater;

NCM t THEREFORE, BE IT RESOLVED BY Tim CITY COMMISSION OF THE '
CITY OF CLEARWATER, FLORIDA, IN SESSION DULY AND REGULARLY
ASSEMBLED, AS FOLLOWS:
1. That Barnett Bank of Pinellas County, N.A. is authorized to accept

requests from Gallag~er Bassett Insurance Services 'for the Electronic Transfer of
funds on the City of Clearwater Self Insurance Program Account.

2. That certified copies of this Resolution be forwarded forthwith to
Barnett Bank of Pinellas County, N.A. and said copy ahall be the official
authorization of the City of Clealvater to said Bank to accept requests for the
Electronic Transfer of funds from Gallagher Bassett Insurance Services immediately.
3. That this Resolution shall be effective immediately upon its
