CONTRACTOR'S SERVICES
Bla-Spec agrees to collect and removo from the Customers premises. !no81 and/or dispose of all blo.medical we.te
generaled by Customer 81 Ihe retes sel Ionh above and in accordallCll With the terms of Ihis agreelTlllnt. The
Customer will be provided a manifest and facility name where all waale is incinerated. The ~IO~ be removed
from Ihe Customer's pickup 10c81ion In aceordallce with the followlnll.chedule: f\l~ ~y y.~

Bio-Spec has obtained all necessary permils, licenses. zoning and oIher federal, slllte or local /Iulhor/zelions
reqllired to perform services hereunder, and, upon reque~ of Customer, Blo-SpeC will turnlllll cople, thereof \0
Customer.
CUSTOMER'S OBLIGATIONS
In consideration for the services provided end the rates charglld by Bla-Spec, the Customer agrees 10 Ull8 on an
exclusiVe bssis, the services described herein by Bio-Spec for bio-medicat waste mllterial generllted at the above
Customer 10caCion(s). By execution of this Agreement, CuslOmer ",prasenls and warrattlS IhIIl cuslomer III not
currently a party to any other ag......ment with an independent contractor or other patty wlIIch prgvlded the same or
similar services as contemplated 10 be providecllly Bio-Spec Ilerein.
Customer is solely respo/lsible for properly and lawfully packaging, labeling and sloring bio-medlcal Wlll'le. Customer
shatl store biG-mediCal wllste in contalne... lIuppUed or approved by Bio-S~c. Cuslomer agrees that all conlalners
supplied 10 Customer by Bio-Spec and which carry Bi~Spec identificalion and/or 'abellng on the conteiner aha" be
collected and disposed of only by 8io-$p~. C\.I$tomer agrees that the storage of bio-mediCal waste snail be
confined 10 an area on Customer's premises, /0 whICh only aulllorlzed personne' shall have access. If Culllomer
elects to supply ils own container, Customer will be solely responsible for Ihe apprgpriate con.truction. m8rklng and
labeling of such conlainers, but use of such containers shall be subjecllo Bio-Spec's approval.
RECORD KEEPING
Bio-Spec and Customer shall keep and retain ;J40quate books, ...cords and other documentation inclll<llnQ personnel
",cords, correspondence.lnslructions. p/ans.reeeipIS.vouchers.coples of all manirests Bnd packaging records and
any other recon:lS or reports or memoranda. consilltent with and for the periods roquifad by IlIlplicable regulatory
requirements Ilnd guidelines p8t'laining 10 pel"fotmance 01 services required by this Agreement.
SIGNING IN THE SPACE PROVIDED BELOW. THE CUSTOMER ACKNOWLEDGES HAVING READ AND THAT IT
IS BOUND BY THE TERMS AND CONDITIONS ABOVE.
