AGREEMENT FOR SERVICES EXTENSION (3)AGREEMENT FOR SERVICES EXTENSION
THIS AGREEMENT made and entered into on the 1St day of October 2026 by
the city of Clearwater, Florida, a municipal corporation, hereinafter referred to
as the "City", and Tampa Bay Psychology Associates, LLC, hereinafter referred
to as "Professional" is to extend the current Services Agreement entered into
between the City and Professional.
WHEREAS, the City entered into a Services Agreement with Professional on
October 1, 2023 to provide department training, consultation, response to and
debriefing of critical incidents, individual counseling, psychological assessment
and referral services, and other ancillary and supplementary psychological
services in support of the Clearwater Police Department.
WHEREAS, the term of the agreement, which was approved by the Clearwater
City Council, was for two (2) years with an option of three additional one (1)
year extensions thereafter;
WHEREAS, the City and Professional agree to extend the Services Agreement
for an additional one (1) year extension;
NOW THEREFORE, in consideration of the promises and mutual covenants
contained in this Agreement, the Professional and City hereby agree as follows:
The Services Agreement between the City and Professional is hereby extended
for an additional one (1) year. All other terms and conditions of the original
Services Agreement shall remain in effect. Updated fee scheduled below.
