FIRST AMENDMENT TO COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM AGREEMENTFIRST AMENDMENT TO COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) PROGRAM AGREEMENT BETWEEN THE CITY OF CLEARWATER, FLORIDA AND METROPOLITAN MINISTRIES, INC. THIS FIRST AMENDMENT TO CDBG PROGRAM AGREEMENT (this "Amendment") is made and entered into by and between the City of Clearwater, Florida, a Florida municipal corporation, with a principal address of P.O. Box 4748, Clearwater, Florida 33758, (the "City"), and Metropolitan Ministries, Inc., a Florida not-for-profit corporation, with a principal address of 2002 North Florida Avenue, Tampa, FL 33602 ("Metropolitan Ministries" or "Subrecipient"). (The City and Subrecipient shall be referred to collectively as the "Parties"). RECITALS: WHEREAS, the City administers the Community Development Block Grant ("CDBG") Public Services Program (the "Program") funded by the United States Department of Housing and Urban Development ("HUD"); and WHERAS, the City is an entitlement community that receives CDBG funds awarded under the Housing and Community Development Act of 1974, in furtherance of its goal of promoting community development and improvement of public facilities, as further detailed in the Consolidated Plan for Housing and Community Development Programs 2020/2021-2024/2025; and WHEREAS, Metropolitan Ministries is a not-for-profit corporation that provides comprehensive services for the homeless and those at risk of becoming homeless in underserved and impoverished communities; and WHEREAS, the Parties entered into that certain CDBG Program Agreement dated December 2, 2024 (the "Agreement") to utilize Forty -Seven Thousand Twenty -Four Dollars and 00/100 Cents ($47,024.00) in FY 2024-2025 CDBG funds for Salary Support Benefits to enable Metropolitan Ministries to deliver the NeighborHOPE North Greenwood Program; and WHEREAS, these services are an eligible activity under the CDBG program in accordance with 24 CFR §570.201(e). It is a limited clientele activity and meets a national objective as required under 24 CFR §570.208(a)(2); and WHEREAS, Metropolitan Ministries has available the necessary qualified personnel, facilities, materials, and supplies to perform such services and/or carry out such programs for these residents, who are eligible and qualified to received said services and are within FY 2024-25 Metropolitan Ministries, Inc. PS 16180 —151 Amendment Page 1 of 6 the income limits for low -and moderate -income persons as defined by HUD and adjusted annually, and which current income levels are attached as Exhibit "A" and incorporated here in reference; and WHEREAS, the Parties now wish to amend the statement of work to allowgrantfunds to be used for Fringe Benefits; and WHEREAS, salary support shall be for the salaries and wages of staff for hours worked towards activity delivery Pursuant to 2 § CFR 200.431, fringe benefits are allowances and services employers provide to their employees as compensation in addition to regular salaries and wages. Fringe benefits include, but are not limited to social security, employee life, health, unemployment, and worker's compensation insurance (except as indicated in § 200.447 ) holidays and sick leave. WITNESSETH: NOW THEREFORE, in consideration of the mutual covenants and agreements contained herein, and for other good and valuable consideration, the sufficiency and receipt whereof being hereby acknowledged, the Parties agree as follows. 1. Recitals. The recitals set forth above are true and correct and are incorporated herein and made a part of this Amendment. 2. Amended and Restated Sections. Secti on I, Paragraph 4. of the Agreement is hereby amended and restated in its entirety as follows: "4. Statement of Work/Program and Project Description. Subrecipient provides comprehensive services for the homeless and those at risk of becoming homeless in underserved and impoverished communities. The subrecipient will deliver the NeighborHOPE North Greenwood Program. The project will benefit 89 low- to moderate -income City of Clearwater residents according to HUD income guidelines attached as Exhibit "A" (the "Project"). Additional requirements for the Project are attached as Exhibit"C" and made a part hereof by this reference. The budget for this Project (the "Budget") is attached as Exhibit "D" and made a part hereof by this reference. Funds are provided for salary support and fringe benefits for a full-time Case Managerforthe NeighborHOPE North Greenwood Program. The NeighborHOPE North Greenwood Program addresses the needs of households experiencing poverty and family homelessness in Clearwater. The Case Manager (CM) will provide housing FY 2024-25 Metropolitan Ministries, Inc. PS 16180 —1 st Amendment Page 2 of 6 services and community resources to families in North Greenwood who are at risk of becoming homeless. The CM assesses the strengths and opportunities of each family and works with households to develop self-sufficiency goals and create a self- sufficiency plan. The CM connects participants to community resources to address employment, education, health, mental health, childcare, financial wellness, and any other needs related to family wellbeing. When necessary, housing search assistance and referrals to community housing programs are provided to prevent homelessness. Metropolitan Ministries (MM) will provide community navigation, resources, and case management to job seekers who are homeless or at risk of homelessness. MM is an active member of the Pinellas Homeless Leadership Alliance (HLA) and is experienced in using the Pinellas Homeless Management Information System (HMIS) to document client services and assess project outcomes. The project supports the HLA Continuum of Care's Housing First service delivery model. 3. Amended and Restated Exhibits. Exhibit "D" of the Agreement is hereby amended and restated in its entirety as follows: "Exhibit "D" Budget Metropolitan Ministries, Inc. In the event of any conflict or ambiguity between the terms and provisions of this First Amendment and the Agreement, the terms and provisions of this First Amendment shall control the extent of any such conflict or ambiguity. 5. All provisions of the Agreement not expressly amended herein shall remain in full force and effect.
