SPECIAL WARRANTY DEED
THIS SPECIALWARRANTY DEED, is made this 234day of May 2025, by and between the City of
Clearwater, Florida, a Florida municipal corporation whose address is 600 Cleveland Street, Suite 600,
Clearwater, FL 33755 (the "Grantor") and Community Service Foundation, Inc., a Florida not-for-profit
corporation whose address is 925 Lakeview Road, Clearwater, FL 33756 (the "Grantee").
WITNESSETH: That the Grantor, for and in consideration of Ten Dollars ($10.00) and other good and
valuable considerations, the receipt and sufficiency of which are hereby acknowledged, hereby grants, bargains,
sells, aliens, remises, releases, conveys and confirms unto the Grantee, its successors and assigns forever, all the
right, title interest claim and demand which the Grantor has in and to the following described real property, situate
lying and being in the County of Pinellas, State of Florida:
The North 45 Feet of Lot 5 and 6, Block "H", a Corrected Plat of the Clearwater Improvement Co.,
Plaza Park Addition, Clearwater, Fla, according to the map or plat thereof, as recorded in Plat
Book 5, Page(s) 53, of the Public Records of Hillsborough County, Florida, which Pinellas County,
Florida was formerly a part.

The Grantor does hereby warrant the title to said land and will defend same against the lawful claims of
all persons claiming by, through or under the Grantor, its successors and assigns and not otherwise.
The property must be used by the Grantee for the sole purpose of constructing a single-family home to be
sold or rented to a household whose total household income does not exceed one hundred twenty percent (120%)
percent of Area Median Income, adjusted for household size, as published by the United States Department of
Housing and Urban Development for Pinellas County. Grantee agrees to: 1.) commence construction of the
single-family home within six (6) months of the date of this deed, 2.) complete construction of the single-family
home within twelve (12) months of the date of this deed, and 3.) convey the property to an income qualified -
household within eighteen (18) months of the date of this deed (collectively, the "Activities"). If the Grantee does
not complete the Activities within the specified timeframes, the property shall, upon request of the Grantor, revert
to the Grantor via a special warranty deed at no cost to the Grantor. The Grantor shall be entitled to specific
performance among any other remedy available at law or at equity in the event the Grantee fails to reconvey the
property at the Grantor's request. The Grantor retains the unilateral discretion to extend these timeframes as it
determines appropriate and may notify the Grantee in writing of such an extension.
THIS DEED IS SUBJECT TO THE FOLLOWING: Applicable zoning restrictions and to easements,
reservations and restrictions of record, if any, which are not specifically reimposed or extended hereby, and to
taxes for the year 2025 and subsequent years, if any.
TO HAVE AND TO HOLD the same together with all and singular, the appurtenances thereunto

belonging or in anywise pertaining all estate, right, title, interest and claim whatsoever of the Grantor, either in
law or in equity, to the only proper use and behoof of the Grantee, its successors and assigns, forever.
