The Chair called the meeting to order at 9:40 a.m. at City Hall.
To provide continuity for research, items are in agenda order although not necessarily discussed in that order.
ITEM #2 - Approval of Minutes
Member Breest moved to approve the minutes of the regular meeting of October 4, 1995, as recorded and submitted in written summation to each member. The motion was duly seconded and
carried unanimously.
ITEM #3 - Old Business
a) Light Duty Policy (Cont'd)
b) Timothy DeBord - Firefighter - Fire Department - Job-connected Disability Pension - Chair Hooper moved that Timothy DeBord be granted a job-connected disability pension effective
on a date to be determined based on Mr. DeBord's disability that resulted from on-the-job injuries which are described as degenerative disk disease complicated by a slip and fall injury
which has caused permanent damage to his lower back specifically L5-S1 making him permanently unable to do his job and documented by medical statements and the following physicians:
Francisco Gomes, M.D.; Clinton B. Davis, M.D.; Robert Gruber, D.O.; Joseph Papia, D.C.; and other supporting medical documentation attached to his file. The motion was duly seconded
and carried unanimously.
c) Gary Milarck - Marine Facility Operator, Marine Department - Job-connected Disability Pension - Ms. Ford will schedule an appointment for an IME with the specialist Mr. Milarck chooses
from the City's list.
d) William Matlin - Mechanic II, Fleet Maintenance - Job-connected Disability Pension - Not present - Case pending
e) Linda Traub - Container Maintenance Worker, Solid Waste Department - Job-connected Disability Pension - As IME does not specify permanency or degree of disability
limitations, Ms. Traub's attorney, John Tucker, will contact Dr. Sirna for a clarifying letter to be submitted for the November 1, 1995 meeting.
f) Cora Bennett, widow of Ernest Bennett -Solid Waste Worker, Solid Waste Department - Survivor's Pension - Not present - Case pending
g) Leo Thompson - Spray Technician, Parks & Recreation Department - Job-connected Disability Pension - Not present - Case pending
h) Robert Batten - Equipment Operator III, Public Works Department - Job-connected Disability Pension - File awaiting two items. IME not addressed yet.
i) Gary Lutman - Mechanic II, Fleet Maintenance Division, General Services Department - Job-connected Disability Pension - Chair Hooper moved that Gary Lutman be granted a job-connected
disability pension effective on a date to be determined based on Mr. Lutman's disability that resulted from on-the-job circumstances which are described as multiple including the left
hip, upper extremities, back and knee; all these injuries making him permanently unable to perform his job and documented by numerous medical statements of Gordon Zuerndorfer, M.D.;
Gary Muscovitz, M.D.; Michael Evans, M.D.; Michael Thompson, M.D.; and Mark Monical, M.D. with accompanying dates of all the doctor visits. The motion was duly seconded and carried
unanimously.
ITEM #4 - New Business
a) Willie Frazier - Equipment Operator III, Parks & Recreation Department - Non-job-connected Disability Pension - Chair Hooper indicated the rules state action on a pension requires
consideration during two meetings. This issue will be voted on November 1, 1995.
The Committee recessed from 10:47 to 10:54 a.m.
b) David Bossard - Firefighter, Fire Department - Job-connected Disability Pension - A letter from a medical doctor is forthcoming. Ms. Ford will schedule an appointment for an IME
with the specialist Mr. Bossard chooses from the City's list.
The Committee recessed from 11:00 to 11:05 a.m.
ITEM #5 - Approval of New Members
Member Breest moved to accept the following employee(s) into membership in the Pension Plan:
