ADDENDUM TO SERVICE AGREEMENT (2)ADDENDUM TO SERVICE AGREEMENT
THE SERVICE AGREEMENT (the "Agreement") DATED December 22, 2022, and
effective as of January 1, 2023, made and entered into by and between COMMERCIAL RISK
MANAGEMENT, INC. (referred to herein as the "Company") and the CITY OF CLEARWATER
(referred to herein as the "SELF-INSURED") is hereby amended to include the administration of
the Cancer Coverage for Firefighters Florida Statute 112.1816 to the self-insured program. The
Service Fee for the administration of 112.1816 for the (15) Fifteen -month period
January 1, 2025 through March 31, 2026 will be $9,375.00 or $625.00 per month.
Commercial Risk Management, Inc. will review, research and determine if diagnosis falls under
Section 112.1816 Florida Statutes for entitlement of benefits (copy of Cancer Coverage for
firefighters 112.1816 is attached and is part of this addendum). Commercial Risk Management,
Inc. will issue payments in accordance with Statute 112.1816, as approved by the City of
Clearwater.
Any renewal of the term of this Agreement shall be documented by an instrument in writing signed
on behalf of both the Company and the Self -Insured.
IN WITNESS WHEREOF, the Company has caused this Addendum to be executed by its
undersigned officers to be fully authorized this 6th day of November 2024 but effective as of
January 1, 2025.
