Dear IVIS. Hinson,
By mutual agreement and on behalf of the City of Clearwater, I would like to extend the Agreement with
the Life Force Cultural Arts Academy, Inc.for a period of five (5)years commencing on the 11 day of
October 2023 and continuing until the 30'day of September 2028. This extension is contingent upon
receipt of all required documentation and subject to the same terms and conditions provided in the
original Agreement dated July 22, 2018, including any renewals of current program insurances as
necessary during this extension period.
Since mutual agreement is required, please sign and return as soon as possible.
