FIRST AMENDMENT TO AGREEMENT FOR SERVICESFIRST AMENDMENT TO AGREEMENT FOR SERVICES
THIS FIRST AMENDMENT made and entered into on the 9 _ day of August
2023, by and between the CITY OF CLEARWATER ("City"), a Florida municipal
corporation, P.O. Box 4748, Clearwater, Florida 33758 and Tampa Bay
Psychology Associates, LLC ("Professional"), a Florida corporation, 111
North Belcher Road, Suite 101, Clearwater, Florida, 33765.
WHEREAS, the City and Professional entered into an Agreement for Services
September 15, 2022 for professional services involving the Clearwater Fire &
Rescue Department. That agreement expires on August 31, 2023;
WHEREAS, the City and Professional anticipate City Council approval of
a new agreement for services with an effective start date of October 1,
2023;
WHEREAS, the City and Professional agree to extend the Agreement
for Services for an additional one month, from September 1, 2023,
through September 30, 2023, and to increase the not -to -exceed amount
for an additional $7,000.00, from $70,000.00 to $77,000.00;
NOW THEREFORE, in consideration of the mutual promises stated herein and
other good and valuable consideration, the Parties agree that the above terms,
recitals, and representations are true and accurate and are incorporated
herein by reference, and the Parties further agree as follows:
1. The Agreement for Services will be extended for an additional one month,
from September 1, 2023, through September 30, 2023, and the not -
to -exceed amount will be increased an additional $7,000.00, from
$70,000.00 to $77,000.00.
2. All other terms and conditions of the original Agreement for
Services shall remain in effect.

IN WITNESS WHEREOF, each Party represents that it has read this First
Amendment and agrees to perform in accordance with the terms and conditions
contained herein. Each Signatory to this First Amendment warrants by affixing his
or her signature below that he or she is duly authorized to bind the Party whom
such Signatory represents.

To get our agreement on a Fiscal Year basis, your approval is needed to extend the Tampa Bay
Psychology Associates LLC agreement by one month through September 30, 2023, and add $7,000.00 to
contract no. 901735 bringing the contract total to $77,000.00.
1. This item was approved by Council 09/15/2022: Link to agenda: 09.15.22 Agenda
2. CFR will be bringing a new agreement forward for Council's approval in the near future.
