AGREEMENT FOR SERVICES EXTENSIONAGREEMENT FOR SERVICES EXTENSION
THIS AGREEMENT made and entered into on the 'qv.' day of—Al, 2023 by
the City of Clearwater, Florida, a municipal corporation, hereinafter referred to as the "City",
and Tampa Bay Psychology Associates, LLC, hereinafter referred to as "Contractor" is to
extend the current agreement for services entered into between the City and Contractor
entered into August 1, 2021.
WHEREAS, the Agreement for Services and all extensions expire July 31, 2023; and
WHEREAS, the City and Contractor anticipate City Council approval of a new service
contract with an effective start date of October 1, 2023; and
WHEREAS, the City and Contractor agree to extend the Agreement for Services for an
additional 2 months from August 1 through September 30, 2023 to bridge the gap between
the expiration of the new Services Agreement and effective start date of the new contract
term;
NOW THEREFORE, in consideration of the promises and the mutual covenants contained in
this Agreement, the Contractor and City hereby agree as follows:
The Service Agreement between the City and Contractor is hereby extended through
September 30, 2023. All other terms and conditions of the original Agreement for Services
shall remain in effect.
