Box 4748 Clearwater, Florida 33758 Dear Mr. Hallos, This letter serves to amend the FY22 Agreement between City of Clearwater - Charting the Course for Youth and the Juvenile Welfare Board of Pinellas County (JWB). On April 14, 2022, the Board approved an award in the amount of $10,000 for workforce stabilization. This is Amendment #3 to the Agreement and increases the total allocation from $399,095 to $409,095. Upon full execution of this document a budget amendment may be submitted to reflect the allocation increase for costs beginning on June 1, 2022. Further, this letter amends the Agreement to include the following special Pinellas County Commissioner condition: Dr. Michael. A. Quarterly reports shall be uploaded to JWB's Secure Portal by the 20th of the month following the reporting period (te. January 20th, April 20th, July 20th, and October 20th). The report for June 1, 2022 - September 30, 2022 may be included in the FY22 fourth quarter report which will be due October 20, 2022. The reports and calculations shall be made using the rate calculators provided in the Workforce Stabilization application. All other terms and conditions of the Agreement shall remain the same.
