CONTRACTUAL PROVISIONS
Customer acknowledges that this contract constitutes the entirety of the work to be completed and that it supersedes all
previously issued contracts, revisions, addendums, and verbal agreements. Goods and services not listed in this contract
will not be provided unless and until agreed upon in writing by the parties in an addendum contract. This contract is valid
for sixty (60) days from the date of execution indicated by the signature date of the PRIDE HVR manager below. PRIDE HVR
makes no guarantees, expressed or implied, as to the completion or delivery date of contracted projects. As such, delivery
dates are estimates only and subject to change. Production scheduling is dependent upon the obtainability of parts and
materials, institutional matters beyond PRIDE HVR's control, and unexpected complications in the fabrication or renovation
process. Emergency vehicle projects take precedence in scheduling which may also affect the completion and delivery dates
of other projects.
REFURBISH COMMAND CENTER
a. Comprehensive inspection upon arrival. Irregularities and concerns documented and communicated to
customer.
b. Schedule Project Management Reviews "PMR" as needed.
c. Quality Assurance inspection upon completion of work.
d. Furnish equipment operational manuals.
e. Provide customer with training on vehicle and equipment usage at the time of delivery.
EXTERIOR — PAINT
a. Remove parts and equipment where practicable to ensure surfaces underneath are painted.
b. Mechanically and chemically degrease and clean the exterior and under carriage of the vehicle for
finishing. Clean and seal roof as needed.
c. Remove existing graphics, and repair minor rust spots and body damage.
d. Sand, seal, and prep vehicle for paint.
e. Paint the exterior of the vehicle, to include doorjambs, Clearwater P.D. BLUE using a base -coat / clear -
coat finish (customer to provide exact color).
i. The interior of compartments and boxes shall not be painted.
f. Paint exposed portions of the frame and undercarriage BLACK.

a. Fabricate and apply a custom vehicle graphics package consisting of reflective lettering and agency logo.
Customer shall provide all artwork.
a. Purchase and install one (1) HDTV capable omni -directional antenna. Installation shall include all
equipment and wiring necessary to connect the antenna to each monitor.
b. Replace existing green awning fabric with a fabric that is similar to the Clearwater P.D. BLUE of the
vehicle.
c. Replace existing back-up camera system. Installation includes a 9" monitor installed in the cab.
INTERIOR — CAB
a. Remove existing grey carpet on walls and ceiling. Replace with black carpet.
b. Remove existing fluorescent lights and install new Whelen Super -LED red/white changeable 8" round
lights.
c. Replace existing white 120VAC outlets and covers with black 120VAC outlets and covers.
INTERIOR -- FRONT ROOM
a. General.
b. Driver Side.
i. Demo room to include base and upper cabinetry, electrical, and wall, ceiling and floor
coverings. Removed items shall be saved for repurposing in the vehicle as needed.
ii. Remove passenger side dividing wall (between workstation and scientific equipment).
iii. Cover the existing walls, ceiling, and floor in black carpet where practicable.
iv. Install Whelen Super -LED red/white changeable 8" round lights to ensure adequate
lighting for the room's intended use.
v. Paint existing A/C shrouds black and reinstall.
vi. Replace existing white 1.20VAC outlets and covers with black 120VAC outlets and covers.
i. Build out wall to create recesses for wall -mounted HDTVs.
ii. Design, fabricate, and install one (1) end table utilizing the existing base cabinet
(modified to fit space) and existing countertop (modified to fit space) to cover water
heater.
a. Base cabinet and door shall be prepped and painted slate grey.
iii. Purchase and install three (3) 50" Samsumg LED HDTVs (or equivalents) on the wall.
Installation shall include all wiring and connectivity (Coax, HDMI, USB, etc.) to ensure
monitors are interactive with all other monitors in the vehicle.
iv. Install customer -provided laptop docking stations, one (1) per workstation. Installation
shall include under -cabinet 120VAC outlets for concealing power cords.
v. Design, fabricate, and install one (1) bench seat. The base shall be painted grey to match
the end table. The seat cushion shall be upholstered in black vinyl similar to the bench
seats in the rear room.

a. The seats shall be hinged and supported by gas shocks to allow access
to storage underneath.
c. Passenger Side.
i. Modify existing base cabinet by removing the right door to create addition leg space.
ii. Existing upper and base cabinetry shall be prepped and painted slate grey. Dry erase
boards installed in doors shall not be painted.
iii. Install upper cabinetry from driver side in the space created by removing dividing wall
and scientific equipment. Upper cabinetry shall be modified to fit and painted slate grey.
Dry erase boards installed in doors shall not be painted.
iv. Install countertop from driver side in the space created by removing dividing wall and
scientific equipment. Countertop shall be modified to fit (note: a seam will be visible
where the two countertops are joined).
v. Purchase and install four (4) 32" Samsung LED HDTVs (or equivalents) on the wall
creating four workstations. Installation shall include all wiring and connectivity (Coax,
HDMI, USB, etc.) to ensure monitors are interactive with all other monitors in the
vehicle.
vi. Purchase and provide four (4) rolling task chairs. Installation shall include tie -downs for
use while vehicle is in motion.
vii. Purchase and install 120VAC outlets with USB chargers at each workstation.
viii. Install one (1) customer -provided PD radio base unit. Installation shall include mounting
the antenna.
1. Customer to deliver base unit and antenna (NO MIC) upon delivery of vehicle.
INTERIOR_- CENTER Room
a. General.
i. Demo room to include base and upper cabinetry, electrical, and wall, ceiling and floor
coverings. Removed items shall be saved for repurposing in the vehicle as needed.
ii. Cover the existing walls, ceiling, and floor in black carpet where practicable.
iii. Install Whelen Super -LED red/white changeable 8" round lights to ensure adequate
lighting for the room's intended use.
iv. Paint existing A/C shrouds black and reinstall.
v. Replace existing white 120VAC outlets and covers with black 120VAC outlets and covers.
b. Driver Side.
i. Existing cabinetry shall be prepped and painted slate grey. Dry erase boards installed in
doors shall not be painted.
c. Passenger Side.
i. Existing upper and base cabinetry shall be prepped and painted slate grey.
ii. Purchase and install one (1) LED under -cabinet task light.
iii. Replace existing microwave with a new black equivalent.
iv. Purchase and install one (1) black paper towel dispenser.
d. Restroom.
i. Demo restroom area to include cabinetry, electrical, plumbing, and wall and ceiling
coverings. Removed plumbing fixtures shall be saved for repurposing in the vehicle.

ii. Cover the existing walls, ceiling, and base cabinet in grey FRP fiberglass panels with
brushed metal trim.
iii. Install Whelen Super -LED red/white changeable 8" round lights to ensure adequate
lighting for the room's intended use.
iv. Purchase and install one (1) black paper towel dispenser, one (1) mirror, one (1) toilet
paper holder, and one (1) soap dispenser.
a. General.
b. Driver Side.
i. Demo room to include base and upper cabinetry, electrical, and wall, ceiling and floor
coverings as needed. Removed items shall be saved for repurposing in the vehicle as
needed.
ii. Cover the existing walls, ceiling, and floor in black carpet where practicable.
iii. Install Whelen Super -LED red/white changeable 8" round lights to ensure adequate
lighting for the room's intended use.
iv. Paint existing A/C shroud black and reinstall.
v. Replace existing white 120VAC outlets and covers with black 120VAC outlets and covers.
vi. Replace existing window coverings with black blinds.
i. Existing upper cabinetry and dry -erase board frames shall be prepped and painted slate
grey.
ii. Purchase and install three (3) LED under -cabinet task lights.
iii. Replace existing 120VAC outlets on the front of the bench seats with 120VAC outlets
with USB chargers.
c. Passenger Side.
d. Rear Wall.
i. Existing media cabinet and dry -erase board frames shall be prepped and painted slate
grey.
ii. Purchase and install at the media cabinet one (1) matrix switch to connect all of the
monitors installed in the vehicle.
iii. Replace existing television with a Samsung LED HDTV of similar size.
iv. Install two (2) 120VAC outlets on the front of the bench seats.
v. Install one (1) customer -provided PD radio base unit. Installation shall include mounting
the antenna.
1. Customer to deliver base unit and antenna (NO MIC) upon delivery of vehicle.

CUSTOMER AGREEMENTS - PLEASE READ

* Plus applicable sales tax.
The removal of windshields, back glass, quarter glass, and reveal molding carries the inherent risk of breakage.
Customer agrees to hold harmless PRIDE HVR for glass breakage during the removal process — if applicable — and
acknowledges that liability for breakage, and additional costs of replacement, rest solely with Customer if this service is
requested.
Sometimes, worn, broken, or damaged parts and materials are obscured or not evident upon initial evaluation.
This contract is based on PRIDE HVR's assessment of project details provided by Customer (typically photographs) prior to
the creation of the contract. Therefore, Customer agrees that this contract does not cover additional costs of repairing or
replacing latent damages or defects only discoverable after the work has been started. As such, quotes on parts, labor, and
services are current, but subject to change.
Customer agrees that equipment, parts, materials, appliances, hardware, etc. removed during the renovation
process will be discarded unless otherwise specified in writing by Customer prior to the completion of work.
Indemnification Clause: PRIDE Enterprises has extended sovereign immunity under section 946.5026 and 768.28,
Florida Statutes, and, therefore, as a matter of Florida law, cannot agree to an indemnification or hold harmless clauses in
any contract with a private company or individual. In addition, the state of Florida will not allow an additional insured on
PRIDE's coverage, nor will the State include a waiver of subrogation in favor of any private company or individual.
Warranty: One (1) year for paint. One (1) year for craftsmanship. Parts and materials per existing manufacturer's
warranty.
Terms are NET THIRTY (30) from the date of invoice.
PRIDE HVR is a secured facility that does not permit any type of communications equipment, firearms, or weapons
on the premises. Customer agrees to remove all communications equipment, firearms, and weapons from the vehicle(s)
prior to transfer to PRIDE HVR. PRIDE HVR is not responsible for personal property left in vehicle(s).
By signing below or by submitting any written or electronic authorization, payment, deposit, or purchase order,
Customer authorizes PRIDE HVR to perform the scope of work specified herein and agrees to abide by and be bound to
the terms and conditions above. PRIDE HVR's commencement of work shall not constitute acceptance of any additional
or different terms and conditions proposed by Customer not previously agreed to in writing, and PRIDE HVR's failure to
object to provisions contained in any document generated by Customer shall not be construed as a waiver by PRIDE HVR
of PRIDE HVR's terms and conditions, or an acceptance of any terms and conditions of Customer not previously agreed
to in writing, which are hereby rejected by PRIDE HVR.
