Myrtle Ave., Clearwater, Florida, between the hours of 8:30 a.m. and 4:30 p.m. Monday thru Friday, on AUGUST 1, 2007, until no later than close of business three days preceding the bid opening. A charge of $80.00 none of which will be refunded, will be made for each set. The work for which proposals are invited consists of renovations of the National Guard Armory for Special Event Offices, work includes but not limited to minor demolitions, interior partitions, new electrical services & fixtures, new HVAC systems, new plumbing facilities and related site works. Project is located 706 N. Missouri Ave. Clearwater, Florida 33756. A MANDATORY Pre -Bid Conference for all prospective bidders will be held on WEDNESDAY, AUGUST 16, 2007 at 10:00 a.m. ON SITE at NATIONAL GUARD ARMORY 706 N. MISSOURI AVE. CLEARWATER, FLORIDA, 33756. Representatives of the Owner and Architect will be present to discuss this Project. Sealed proposals will be received by the Purchasing Manager, at the Purchasing Office, located at the Municipal Services Bldg., 100 So. Myrtle Ave., 3`d Floor, Clearwater, Florida 33756-5520, until 1:30 P.M. on THURSDAY, SEPTEMBER 6, 2007, and publicly opened and read at that hour and place for NATIONAL GUARD ARMORY RENOVATIONS (06 -0036 -PR) drawings prepared by Fowler Architects & Associates. A complete bidders package containing plans, specifications, bond forms, contract form, affidavits and proposal form is available only to City pre -qualified contractors in the construction category of building structure construction with a minimum pre -qualification amount of $400,000.00. Contractors, suppliers, or others who are not pre -qualified but who may be interested as a possible subcontractor, supplier, etc., may purchase a "Subcontractor" package consisting of plans, specifications, and pay items worksheet. A 10% bid bond is required for all City of Clearwater projects. The right is reserved by the City Manager of the City of Clearwater, Florida to reject any or all bids. The City of Clearwater, Florida William B. Home II, City Manager SCOPE OF WORK 1.1 SCOPE DESCRIPTION Project: National Guard Armory Remodeling - Project No. 06-0036-PR Scope of Work: 1. Renovations of the National Guard Armory for Special Event Offices and minor work on the Warehouse work includes but not limited to minor demolitions, interior partitions, new electrical service and fixtures, new HVAC systems, new plumbing facilities, fire sprinklers system, and related site works. 2. Drawings have been submitted to City of Clearwater - Planning & Development Services Department for Building Permit No. BCP2007-04092, successful bidder will be required to provide the following information for the building permit. a. State Contractor's License b. Name holder of the License c. Occupation License d. Pinellas County Licensing Board Number e. There is no fee for Building Permit but will be required to collect the building permit from Planning & Development Services Department. 3. Application has been submitted to Southwest Florida Water Management District (SWFWMD) Permit and there will be no fee for this permit. 4. Clearing and grubbing permit has been submitted to Land Resources in the Planning & Development Services Department and no fee for this permit. Contractor will be required to collect this permit. 5. Bidders shall note Audio/Video Tape Of Work Areas - by Contractor has been checked off as part of the specification requirement of this project. 6. Bidders shall note specifications have been provided by the City of Clearwater and the Architect conflict between plans and specifications contractor shall inform in writing during the bidding process to resolve these issues. Should conflict arise after contract has been awarded the City of Clearwater article 35 shall govern. 7. Contractor shall secure his construction area at all times. 8. Contractor will be required to provide sanitation facilities for his staff during the duration of this project. Utilization of City of Clearwater restroom will not be available during the construction of this project. 9. City will continue to utilize the parking area during the construction period of this project. A designated lay down, or staging, area with-in the compound will be provided to the contractor. Contractor shall coordinate related site work with the Parks & Recreation Department regards to closure of both site entrances access will not be allowed simultaneously one entrance access shall be open at all times. Contractor shall provide two-week notice when site entrance access disruption will occur. 10. Temporary electrical service will be provided during the construction of this project. Water shall be responsibility of the contractor. If electrical point of connection not adequate during the construction of this project the contractor shall make his own arrangements for electrical services. 11. Contractor shall provide Best Locking System with City of Clearwater master keying system. 12. Owner will provide vertical benchmark on site for contractor's use. 13. Contractor shall note City of Clearwater - Scope of Work Check List only sections check off will apply to this project even though the entire City of Clearwater Technical Specifications are provided. 3 14. Reports dated February 2, 2006 and December 12, 2006 prepared by Apollo Environmntal, Inc. are attached at the end of the technical specifications of this document are Asbestos Survey Environmental, Inc. The contractor shall also be aware the City has completed asbestos removal however the contractor shall review letter dated January 31, 2007 and shall adhere to the contents described in this letter. The contractor is officially informed three areas of asbestos remains within in the facility buildings, for locations see report dated February 2, 2007, tables 'Asbestos Containing Materials Summary Table and 2006 Supplement Sampling, Location and Analysis Summary of Suspect Materials. The three areas are labeled (1) HA-1-2006, soft caulk on exterior window frames, (2) HA-6-2006, 4" pipe elbows and fittings inside south kitchen wall, and (3) HA-11 & 12-2006, assumed roofing under the rubberized roofing on the pitched and flat roofs. The Parks and Recreation Department contracted with Cross Environmental Service to demolish the existing asbestos and other additional items removed. See Architects Drawing Sheets: Demo-1 and Demo-1.1. These two drawings are provided for information purposes and the contractor is not required to perform any work on these two drawing sheets. The contractor shall also review form provided by the Pinellas County Department of Environmental Management "Attention Building Owner and Contractors" and shall adhere to all asbestos regulations on this form. Reports prepared by Apollo Environmental, Inc. and Cross Environmental Services shall be on site at all times during the duration of construction of this project. 15. Others will perform galvanized chain link fence all item associated with this work. This work is located on the ground floor see sheet A-1.1, and mezzanine see sheet M.1. 16. Others will perform installation of the fire alarm & security system installation. Requirement in this scope of work the contractor shall provide all conduit and junction boxes to receive the fire alarm and security system. 17. Others will perform the telephone, data and cable television system installation. Requirement in this scope of work the contractor shall provide all conduit and junction boxes to receive the telephone, data and cable television system. 18. Others will perform the site perimeter fencing including gates, landscaping and irrigation system installation. However the general contractor will be required to provide electrical power source for the irrigation controller as located on the irrigation plans. 19. Warehouse building two existing entrance 3'-0"x6'-8" doors to be removed and replaced hollow metal frame and metal door include painting doors. Doors to swing out with vision panel, kick plate interior only, and door closure w/hold open feature with Best keying system. See detail A on sheet A-1.1 for door elevation. 20. Warehouse Building has 11 existing windows contractor to provide new 2'x2' alum. Window frame with fixed Plexiglas typical for all windows. See attached end of this section_ (See specification document) 21. The owner will provide provided the 8" wet tap, 6" gate valves, and install the 4" double check valve backflow device and fire hydrant. On the down streamside 4" double check valve backflow device will be capped off at this point. The contractor to install all materials down stream from the double check valve backflow device for fire protection of the National Guard Armory building. Contractor shall note he will be required to provide the Fire Department Connection (FDC) within 10' of the proposed fire hydrant installed by the City on Eldridge Street. Provide 4" piping run from the FDC to the 4" riser on the down streamside of the fire sprinkler system butterfly valve in the room where the building riser is located. 22. Included in the contractor will be the installation of the Knox-box provided by the owner. The Knox-box location will be near the front door of the facility as directed by the owner. 23. Others will perform trees to be removed from site shown on sheet 1 of 3, titled Site Plan - Permit. 24. Contractor will be responsible for construction of trash enclosure and City of Clearwater standard detail has been attached as part of this addendum. Concrete slab shall be a minimum of 6- inches thick reinforced with weld wire mesh, 3,500 PSI concrete with commercial fiber mesh. 4 Concrete 6' diameter concrete filled scheduled 40 galvanized metal pipe bollard shall be equal distance apart, 24-inches below and 48-inches above finish grade of concrete slab. Bollards shall be pickled, primed and with two coats semi gloss yellow paint. Contractor shall provide paint samples to owner for approval. Contractor shall note a '/2" sand stucco finish with a minimum of one scratch coat will be required. Painted surface will be required of color matching building color; a minimum of one coat of primer and two coats of exterior semi gloss paint shall be required on all surfaces. Trash enclosure gates shall be white vinyl commercial grade PVC and hung with heavy duty hinges with drop bars for both gates with holes drilled in concrete to keep gates opened and closed. 25. The contractor will be required to excavate removal of gravel/dirt around the perimeter of the fenced in property as shown on the Site Plan - Permit drawing, sheet 1 of 3. The excavation shall be 1 foot deep and 10 feet wide on Eldridge and Missouri, and 20 feet wide on Seminole the contractor shall dispose of this material off site. The Landscape Plan, sheet on of three identifies the landscaping bed buffer surrounding the fenced in property. The landscape plan is provide only the contractor will not be required to perform the landscaping, which will be performed by others. Upon completion of excavation the excavated area shall be filled with planting topsoil. Sample of topsoil shall be provided to the owner for approval. This work shall be performed as one of the final items of the contract period of project. It shall be also noted dimension on excavated area are shown different on the drawing this note shall govern as to the correct dimensions for landscape bed removal and replacement. 26. Contractor shall review Construction Notes shown on Site Plan - Permit drawing sheet 1 of 3 revision date 7/14/07 and perform these task as part of this contract including all other requirements as shown on this drawing. 27. It shall be noted layout of parking is shown on the Site Plan - Permit drawing for information to the Building Department to obtain the building permit, and only provided as information purposes to the general contractor. The contractor's responsibility is to construct employee concrete parking spaces and associated work such as striping, concrete curbs etc... 28. Contractor shall remove existing fence and dispose off site as the final phase of the project and shall coordinate with the owner to installation of the new fencing. 29. The contractor shall be required to provide conductors to security lighting fixtures mounted on the Armory building and make these fixtures operational. Manufacture of light fixture, model no., fixture specifications, and breaker amperage will be provide in Addendum No. 2. 30. Addendum No.1, Scope of Work, 1.1, line 20, states that the Warehouse Building has eleven (11) windows, while t Bidders Proposal, Base Did A, item 4, calls for "Provide fourteen (14) new 2' x 2' aluminum window frames wi fixed Plexiglas". Which is the correct quantity? Answer: Count is13. General Contractor to remove existing air condition units frame out as call for by the detail. It noted the exact size may be larger than 2'x2' and contractor to confirm size. 2. Reference drawing A-1.2, detail '11/A-1.2: We do not understand the "Note: Existing window and frame to remail typical". How do you put a 3-1/2" metal stud in the opening with a window in place? Answer: It is the intention th the stud/plaster cover for the windows will project beyond the face of the existing and offer some architectural relic for the facade. 3. Reference Architects Technical Specifications, Section IV: A. Section 07193 - Elastic Sheet Vapor Barriers is nc included in our copy of specifications. Answer: Provided attached specification with this addendum Item No. 7 1. This is a reminder the fire alarm system will be installed by city's fire alarm monitoring company Maire Company Base bid in this contract is for the general contractor to install all conduits, junction boxes at appropriate locations tc where the fire alarm panel is to be located. The three areas are labeled (1) HA -1-2006, soft caulk on exterior window frames, (2) HA -6-2006, 4" pipe elbows and fittings inside south kitchen wall, and (3) HA -I1 & 12-2006, assumed roofing under the rubberized roofing on the pitched and flat roofs. The Parks and Recreation Department contracted with Cross Environmental Service to demolish the existing asbestos an( other additional items removed. See Architects Drawing Sheets: Demo -1 and Demo -1.1. These two drawings are provide( for information purposes and the contractor is not required to perform any work on these two drawing sheets. The contractor shall also review form provided by the Pinellas County Department of Environmental Management "Attention Building Owner and Contractors" and shall adhere to all asbestos regulations on this form. Reports prepared by Apollo Environmental, Inc. and Cross Environmental Services shall be on site at all times during the duration of construction of this project. 15. (See specification document) 21. The owner will provide provided the 8" wet tap, 6" gate valves, and install the 4" double check valve backflow device and fire hydrant. On the down streamside 4" double check valve backflow device will be capped off at this point. The contractor to install all materials down stream from the double check valve backflow device for fire protection of the National Guard Armory building. 22. Contractor shall note he will be required to provide the Fire Department Connection (FDC) within 10' of the proposed fire hydrant installed by the City on Eldridge Street. Provide 4" piping run from the FDC to the 4" riser on the down streamside of the fire sprinkler system butterfly valve in the room where the building riser is located. 23. Included in the contractor will be the installation of the Knox -box provided by the owner. The Knox -box location will be near the front door of the facility as directed by the owner. 24. A1.1 - rooms on the floor plan indicate a ceiling height of 8', but the finish schedule calls for 9'-4" ANSWER: The ceiling heights are to be as indicated on the plan (a' 8'-0" unless indicated as existing to remain. The ceiling height in rooms 120, 121, and 122 is to be 10'-0". 4. A1.1 - two rooms in the Southeast corner of the building are not identified on the finish schedule and the doors marked #12 and #13 on the door schedule are for the women's bathroom and shower. ANSWER: Those rooms are part of the laundry/ & Warehousing area. The door location on the schedule should read accordingly. 5. A1.1 - will the existing overhead coiling door in Product Storage 102 remain? If not, who will remove and will Owner want door salvaged? ANSWER: The existing coiling door is to be removed and disposed of by the General Contractor. 1 6. A1.1/A 1.2 — please provide specifications for toilet partitions and size of partitions in men's shower area. ANSWER: Please see the attached specification for the toilet partitions. The vertical size of the partitions is the same as the toilet partitions and the width is 30". The mounting frames and hardware is to be the same as the toilet partitions. 7. A l.1 - men's restroom 117 finish schedule indicates that the existing walls are to remain, but window blocking detail 11/A1.2 indicates furring and drywall floor to ceiling. Should the window blocking at this location be only for the window and the existing tile to remain elsewhere on the wall? Similar condition or conflict in Kitchen 119, South wall does not require furring but the window blocking detail goes floor to ceiling, are we to only block the window in the Kitchen? ANSWER: The south wall of the shower room and the toilet are to receive drywall laminated over the existing tile finish. The blocking of the windows in the other areas is to be on the existing surfaces and without additional drywall. 8. A1.1 - the existing walls in numerous rooms are to remain. Will it be acceptable to have surface mounted conduit exposed for switches and outlets? ANSWER: Yes. 9. Sheet A1.3A - the section detail on the mezzanine perimeter is marked as 1/A1.3A. Should this be 4/A1/.3A? ANSWER: Section 1/A -1.3A refers to the typical condition around the perimeter of the storage loft. Sheet P1.4 - Please clarify the plumbing scope to be included with this contract (i.e. demolition, cleanouts, grease trap removal and replacement, etc.)? ANSWER: The existing waste and supply lines that are to he abandoned and are located in partitions or under ground are to be capped and not removed. All supply lines are to be new. All new and replacement tixtures are to be installed with matching surfaces with the exception of wall and floor tiles. These are to receive tile of similar size and uniform color and be a uniform panel of tile to replace the damaged or disturbed areas. Any existing grease trap is to be disconnected, tilled with builder's sand and abandoned. The 750 gal grease trap and associated piping, indicated on the drawings is to be new. Please refer to sheet P1.2 for additional notes for individual tixtures. The carpet tiles are to he: 24"x 24" direct glue down. Adhesive as approved by the carpet manufacturer. Remove or blend tool and die marks and stretch lines into finish. 2. Grind and polish surfaces to produce uniform, directional textured, polished finish indicated, free of cross scratches. Run grain with long dimension of each piece. B. Finish: Manufacturer's standard No. 3 or No. 4 directional polish. C. Protect mechanical finishes on exposed surfaces from damage by applying a strippable, temporary protective covering before shipment. PART 3 - EXECUTION 3.1 INSTALLATION A. General: Comply with manufacturer's written installation instructions. Install units rigid, straight, plumb, and level. Provide clearances of not more than 1/4 inch (6mm) between doors and pilasters, 1/2 inch(13 mm) between pilasters and panels and not more than 1 inch (25 mm) between panels and walls. Clearance at vertical edges of doors shall be uniform top to bottom. Secure units in position with manufacturer's recommended anchoring devices. 1. Secure panels to walls and panels with not less than 2 stirrup brackets attached near top and bottom of panel. Locate wall brackets so holes for wall anchors occur in masonry or tile joints. Align brackets at pilasters with brackets at walls. B. Overhead Braced_and_Floor_Anchored Compartments: Secure pilasters to floor and level, plumb, and tighten. Secure continuous head rail to each pilaster with not less than 2 fasteners. Hang doors and adjust so tops of doors are parallel with overhead brace when doors are in closed position. C.
