PAYMENT & BILLING TERMS
MCCi will invoice one hundred percent (100%) of the software and support upon delivery of software. If services
are included, the balance of the total project will be invoiced upon completion of the proposed professional
services, which may be broken up based on the completion date of specific services. Sales tax will be included
where applicable. Payment will be due upon receipt of an invoice.
Laserfiche software and help files are provided electronically. Hard copy manuals or software are available
upon request, for a $50 additional charge.
Note: MCCi will prorate the support of this additional software to be congruent with your current Laserfiche system's
annual support (LSAP) dates, the purpose of which is to align all software with one support date so maintenance of
your account is simple. MCCi will bill for the actual amount of support, which may be higher or lower than quoted. To
ensure you are budgeting correctly for future renewals, please add the amount listed in the budgetary note above to
your current budgeted renewal amount.
MCCI PROJECT -BASED SERVICES
To determine which services are included with your project, please refer to the Pricing Section.
MCCI PROJECT MANAGEMENT SERVICES
MCCi's Laserfiche certified Team Member administer these services and concentrate on defining
business requirements and the deliverables that follow. The MCCi Team Member will work with the
Client's point of contact to put together a project plan that clearly defines the scope of the Project
Management services, ensuring the Client is prepared for the final project implementation. The total
number of service hours is limited to the total fee quoted, divided by MCCi's current Project
Management rate.
CLIENT CONSULTATION
The assigned MCCi Project Manager will perform a consultation including a review of current
document organization and retrieval practices to determine desired indexing methods, security
rules, and other basic system set up needs. Once this information has been gathered and provided
to the MCCi Project Manager, the basic folder structure, document naming conventions, and
template set-up will be configured prior to onsite training.
REMOTE INSTALLATION AND CONFIGURATION
Software installation and configuration may occur remotely as part of the Project Management
services to ensure the onsite time purchased is focused on the direct objectives. Please refer to the
Pricing Section to determine if the onsite time will include installation and configuration.
REMOTE TRAINING
Project Management services may be utilized for training administrators or users remotely if onsite
training is not included. Please refer to the Pricing Section to determine if training was quoted.

LASERFICHE IMPORT AGENT
Automatically retrieves files stored in a Windows folder and imports them into a Laserfiche
repository, performing OCR as part of the process.
HARDWARE REQUIREMENTS
MCCi will provide necessary consultation upon request, as to the compatibility of current hardware with the Laserfiche System. Changes and
recommendations will be made at the time of consultation. Please keep in mind that these are the minimum system requirements (see below) as
recommended by MCCi and should be considered independently rather than collectively. Additionally, overhead for virtualization has not been
factored in to these requirements. MCCi does not recommend any version of Windows that is approaching or is beyond the "End of Extended

BACKGROUND
The City of Clearwater's Laserfiche Rio implementation is being used throughout the City and
Police Department. IT has identified a Targe volume of data stored on shared drives and SQL
databases from legacy police system that would like to be imported, named and filed into the
current Laserfiche system.
PROJECT OBJECTIVES
• Clearwater will be able to migrate LERS and ReportBeam records from a shared drive
into Laserfiche on a schedule and have them automatically file in Laserfiche.
• Clearwater will be able to migrate data from their current M&I database into Laserfiche
Templates for easy searching.

Structure, and Security
Creation
MCCi Project Manager, while remotely connected to
Clearwater's Laserfiche system, will create the up to three(3)
templates with up to twelve(12) fields on each and a folder
structure for the following system records
- LERS system up to six(6) record types
- ReportBeam system up to four(4) record types
- M&I Data up to one(1) record type
Note: Security considerations will be set based on the type of
record. Security can include at the folder level or a security tag.
#2: Install and Configuration
of Import Agent
MCCi Project Manager, while remotely connected to
Clearwater's Laserfiche system, will work with Clearwater's IT
team to install Import Agent and configure up to two(2) import
profiles that run on a schedule, defined by Clearwater, to
import documents located on Clearwater's shared drive. These
documents will be from the LERS and ReportBeam systems.
The name of the documents will reflect a unique case or report
number. See Appendix A: Detailed Design Specification for
more information.
Note: OCR and PDF conversion to TIFF should be part of all Import
Agent Profiles
#3: Creation of Filing
Workflows
MCCi Project Manager, while remotely connected to
Clearwater's Laserfiche system, will create up to two(2) filing
workflows that will query either the LERS or ReportBeam
databases to retrieve case/report information to fill out the
Laserfiche Template created in Milestone #1 and file according
to the folder structure defined in Milestone #1.
Note: Clearwater will provide the SQL queries and access to both
LERS and ReportBeam needed to retrieve the information based on
the case/report number. If no case/report number is available or
found, manual data entry will be required.

#4: Import M&I Data
MCCi Project Manager, while remotely connected to
Clearwater's Laserfiche system, will create one(1) workflow to
take approximately 1.6 million lines of data in a SQL database
and create a blank record for each line in Laserfiche and apply
up to twelve(12) template fields, setup in Milestone #1,
associated with each line item. The blank record will be filed in
according to the folder structure determined in Milestone #1.
See Appendix A: Detailed Design Specifications for more
information.
Note: Clearwater will provide the SQL queries and access to the
M&l data needed to accomplish this Milestone.
#5: MCCi Alpha Testing
MCCi Testing team will conduct full Alpha testing of the all
workflows built.
#6: Configuration
Knowledge Transfer
MCCi will conduct a knowledge transfer with both Clearwater's
IT and Clearwater's MCCi LAS Representative of what has been
configured and how to monitor the transfer of data.

• MCCi will not be responsible for assigning an external URL for Laserfiche Forms or Web
Access.

CHANGE ORDER PROCESS
As this project progresses, it may be necessary to amend the Statement of Work (SOW). The Client
understands that any change to the original SOW will affect the total hours billed and may extend
the date of project completion. If necessary, the client will send a written request to MCCi outlining a
requested change. MCCi will assess the change and provide the customer with a formal Scope
Change Statement of Work be completing a Change Order request that must be signed by the client.
This revised SOW will include the full scope of the change request, as well as any additional cost that
may be necessary to implement the change. The Change Order template can be found in Appendix
B of this Statement of Work.
It may be necessary to halt work on this project while the client reviews the Scope Change. After
receiving the Scope Change Statement of Work, the client must return a signed approval of the
Scope Change to MCCi before work may proceed on the project.

TERMS & CONDITIONS
Professional Services Pricing and Billing: MCCi has included an estimated amount of professional
services. Actual amounts billed may vary based on the final SOW, change orders, and other such factors.
Billing in excess of the project estimate requires the Client's approval and will be based on the amount of
services utilized. Professional services are billed on a monthly basis.
Business Process Configuration Service SOW Drafting/Approval: The drafting process is a collaborative
effort in which the client must provide accurate data in order for the SOW to thoroughly reflect requirements.
If the data provided by the client is inaccurate or incomplete, Change Orders and an increased budget may be
necessary. The time spent by MCCi's Project Management staff to draft the SOW has been included in the not
to exceed project cost.
Additional Configuration Services: It is likely that Business Processes will change or that enhancements to
the configured Business Processes will be requested after this project is completed. With the exception of
Clients who pre -pay for block professional services time, The Client will be charged based on the then current
MCCi professional service rate for any additional Business Process Configuration Services requested, quoted
to, and approved by The Client.
Software Licensing Needs: Any additional software licensing needs related to this service/process
configuration, have not been considered or included as part of the scope of services. The Client is responsible
for ensuring that the required software licensing is available.
Client Staffing Changes: In the event that the primary Client contacts change during the project, a change
order may be required for training time and other related process changes caused by the staffing change.
MCCi Software Configuration Services: The customer may elect to contract with MCCi to configure the
software. The customer is responsible for testing all software configurations completed by MCCi. By
acknowledging this testing requirement, the customer waives any and all liability to MCCi for any fees,
damages, etc., that could be related to software configurations.
Infrastructure/Hardware: It is The Client's responsibility to ensure that adequate hardware/infrastructure is
in place and capable of handling the extra resources that may be required to support the Business Process
Configuration, related software, etc.
Use of Basecamp: Through the course of this project, MCCi may choose to utilize the third -party service
Basecamp (http://www.basecamp.com) for project management and team collaboration. Documentation and
correspondence exchanged between MCCi and The Client may be stored in Basecamp. The Client
acknowledges that Basecamp is responsible for secure storage of this documentation and agrees that
Basecamp's security guidelines located at http://basecamp com/securiv are acceptable for the storage of The
Client's data and correspondence exchanged with MCCi.
Project Management Hand -Holding Services: This is an optional service. If listed within the deliverables,
included is an "up to" amount of post implementation hand holding services allowing the client continued
access to project team after the delivery of described services. These services are to be leveraged by the client
to discuss any change management procedures, minor modifications & adjustments to process, strategy
meetings and remote (unless onsite is specified) assistance with rollout. The hand -holding period will either
start the day after the last day of training outlined in contract or after the delivery of all services rendered
including any change orders when training was not included. These services will be discontinued (30) thirty
calendar days after the start date.

APPENDIX A - DETAIL DESIGN SPECIFICATIONS
• All LERs record follow a naming convention of "Year-CaseNumber". The Case Number will be
used to query the database in order to retrieve additional metadata. The Year, Type, and Case
number is the most important metadata.
• All ReportBeam records follow a naming convention of "Report Number". The Report Number
will be used to query the database in order to retrieve additional metadata.
• All records to be imported will be PDFs that will be converted to Tiff images and OCR'd when
moved to Laserfiche.
• The M&I data will have 12 columns of information that Workflow will transfer to template fields
on a blank document. No actual record image will be generated.

Change Request Overview
The ******* has requested a change to the services described in the ******* Project Statement of
Work. The change requested will include development and project management services provided
by MCCi resources to develop and unit test software enhancements, etc.
Change Request Impact
MCCi will utilize time from development resources to develop and unit test the specified
enhancements while continuing to work on completion of the deliverables within the ********
=Project Statement of Work. The programming and testing services will be included as an item on
the MCCi task list and prioritized as appropriate by the client.
Change Cost
