Myrtle Ave., Suite 200 Clearwater, FL 33756 RE: Moccasin Lake Interpretive Center, Project No. BCP 2017-10096 As requested, enclosed is the proposal for renovation of Moccasin Lake Interpretive Center submitted in accordance for a Guaranteed Maximum Price of $522,985.00 Scope of Work: Aquarium Tank installation is limited to installation of the Tanks only and does not include any related accessories or equipment such as lids, lights, pumps, etc. 2 Proposal includes tool cleaning of exposed steel in lieu of sandblasting. 3 Mohawk Choice Step LVT is included in lieu of Armstrong Natural Creations LVT. 4 A steel column repair ALLOWANCE of $2,500.00 is included with this proposal. 5 A Material Testing ALLOWANCE of $300.00 is included for cylinder tests. 6 Work is limited to inside of 5' of buildings overhang with the exception of electrical feeds to Duke pedestal. 7 Dumpsters and Hauling are to be provided by OWNER. 8 All permit fees for the project are not included. DeLotto Established 1946 Existing Contract: This proposal is submitted in conjunction with the existing Construction Manager at Risk Services Continuing Contract entered into with the City of Clearwater on August 4, 2016, based on RFO #24-16. If GMP Exceeds $150,000: Per Section III, Article 5.1 of the Contract Specifications, the contractor shall provide to the public entity a certified copy of the recorded bond. Once the City receives a certified copy of the recorded bond, a Notice to Proceed may be issued. Include the attached bond form as well as the Power of Attorney. (If GMP is less than $150,000 a bond is not required.) For work performed, invoices shall be submitted to the City of Clearwater, Engineering Department, Attn: Veronica Josef, Senior Staff Assistant, P.O. Box 4748, Clearwater, Florida, 33758-4748.
