Dear Mr. Chin:
I am pleased to submit the following proposal for the construction management for the Phillies
Memorial Plaza at Jack Russell Field in accordance with plans prepared by Deuel and Associates a
titles Jack Russell Stadium Phillies Memorial Plaza 1-7 1 cover sheet, 2 existing conditions/demolition
plan, 3 site grading and drainage plan, 4 planter wall structural details, 5 planter wall site plan, 6
structural notes, 7 general notes all dated 12-2-17 as more specifically defined below.
Scope of Work:
We will supply construction management, coordination with city's staff, other
subcontractors and vendors, labor, materials and equipment for all concrete foundations and
flatwork, masonry, paver work, waterproofing and sealer, setting of granite stones (provided by
others). Two Additional Light Pole Bases, not shown on plans complete per plans dated 12-2-17
referenced above with the following items specifically excluded see below.
Exclusions
1. Piping storm, sanitary or water.
2. Electrical or low voltage.
3. Removal or repair of any electrical or low voltage found in planter foundaitons area.
4. Earthwork and/or spoils removal.
5. Silt Fencing.
6. Material testing.
7. Paver material.
8. Curbing.
9. Concrete cutting or removal.
10. Bullpen work.
11. Installation or furnishing of wall cap.
12. Monument granite or anchoring hardware.
13. Surveying.

Project Duration shall be 45 calendar days from the date of the Notice to Proceed.
Existing Contract: This proposal is submitted in conjunction with the existing Construction
Manager at Risk Services Continuing Contract entered into with the City of Clearwater on
06/15/2016 based on RFQ #24-16.
If GMP Exceeds 5150,000: Per Section III, Article 5.1 of the Contract Specifications, the
contractor shall provide to the public entity a certified copy of the recorded bond. Once the City
receives a certified copy of the recorded bond, a Notice to Proceed may be issued. Include the
attached bond form as well as the Power of Attorney. (If GMP is less than $150,000 a bond is not
required.)
For work performed, invoices shall be submitted to the City of Clearwater, Engineering
Department, Attn: Veronica Josef, Senior Staff Assistant, P.O. Box 4748, Clearwater, Florida,
33758-4748. Contingency services may be billed only after written authorization is provided by the
City to proceed with those services.
