sodding disturbed areas) has been inspected by authorized representatives of the fawner, Contractor, and
Consultant,and the specified pert of the Project, as indicated above, is hereby declared to be
substantially completed on the above date.
DEFINITION F SUBSTANTIAL NTI PL TIO
Substantial completion is the stage in construction when a project or designated portion
thereof is sufficiently complete in accordance with contract documents so that the Owner
can use the work or portion thereof for its intended use. Items that affect operational
integrity and function of the work,must be capable of continuous use.
A tentative list of items to be completed or corrected, in the substantially complete portion of the
project described above, is appended hereto. This list may not be exhaustive, and the failure to include
an item, on it does not alter the responsibility of the Contractors to complete all the work in accordance
with the Contract Documents.
The Contractor accepts the above Certificate of Substantial Completion of the outlined project area

Jf within one year after the date of Substantial Completion or such longer period of time as may be
prescribed by Laws or Regulations or by the terms of any applicable special guarantee required by the
Contract Documents or by any specific provision of the Contract Documents, any Work is found to be
defective, Contractor shall promptly, without cost to the Owner and in accordance with the Owner's
written instructions; (i) correct such defective Work, or, if it has been rejected by the Owner, remove it
front the site and replace it with Work that is not defective and(ii)satisfactorily correct or remove and
replace any damage to other Work or the work of others resulting therefrom..,
Where dqfective Work(and damage to other Work resulting thereftoM) has been corrected, removed or
replaced under this paragraph the correction period hereunder with respect to such Work will be extended
for an additional period of one year after such correction or removal and replacement has been
satisfactorily completed,

GUARANTEED MAXIMUM PRICE ( GMP)
As requested, enclosed is the proposal for construction of Arlie Avenue Stormwater
Improvements submitted in accordance with the plans issued for this work, and as modified by
clarifications as part of the overall project at Arlie Avenue, for a Guaranteed Maximum Price of
$ 104.555.00 . All work is to adhere to the City's latest standard specifications and details
including Maintenance of Traffic requirements.
Scope of Work:
1. Areas of Work :
Contractor will be working in the vicinity of the Arlie Avenue and Rose Road intersection,
from Station 12+00 on Arlie Avenue to Station 10+80 at the West end of Arlie. Remove
existing storm lift station and storm pipe, perform grading. Construction of concrete swale
will be performed from the west end of Arlie Avenue down to the water's edge of Alligator
Lake. Storm and curb/paving work will also take place on Arlie Avenue as described in above
paragraph and at proposed storm structures S-4 and S-5 at the intersection of Arlie
Avenue and Rose Road.
2. Scope of Work:
Perform corrective storm drainage and paving work at Arlie Avenue/Rose Road as described
further : Demolish existing and construct new storm drain pipe and storm inlets. Demolish and
re -construct curbs and asphalt pavement at this intersection to the west end of Arlie Avenue and
then construct a concrete flume/outfall to drain this area. Finish grade and sod disturbed areas.

Project Duration shall be 60 calendar days from the date of the Notice to Proceed.
Existing Contract: This proposal is submitted in conjunction with the existing Construction
Manager at Risk Services Continuing Contract entered into with the City of Clearwater on August
4.2016, based on RFO #24-16 . For work performed, invoices shall be submitted to the City of
Clearwater, Engineering Department, Attn: Veronica Josef, Senior Staff Assistant, P.O. Box 4748,
Clearwater, Florida, 33758-4748. Contingency services may be billed only after written
authorization is provided by the City to proceed with those services.
