Dear Mr. Wojtkiw,
As requested, enclosed is the proposal for re -roofing and siding of 6 pavilions and
the bait house on Pier 60 submitted in accordance with technical
specs/plans/study/clarifications of this letter/attached line item work, etc., for a
Guaranteed Maximum Price of ($375,267.19). Three hundred, seventy-five
thousand, two hundred, sixty-seven and 19/100 dollars
Scope of Work:
1. Scheduling and coordination with City Staff and Pier 60 Vendors.
2. Provide OSHA Approved scaffolding as required for our scope of work.
3. Removal and reinstallation of turnstile gate.
4. Demolition of all existing metal roofing on bait house and 6 pavilions.
5. Re -roofing utilizing 60 mil self adhered membrane roof underlayment.
6. Remove/replace existing P.T. 2 x 12 fascia and wrap with .032 Kynar standard color
aluminum.
7. Install Gulf Coast 12" 0.C. .032 Kynar standard color aluminum standing seam
roofing/trim with 25 year paint finish and 20 year watertight warranty.
8. Removal of siding material from ceiling elevation above.
9. Remove and replace all exterior soffits with James Hardi vented soffit
10. Provide new James Hardi Ceiling over existing.
11. Replace fire sprinkler heads in exterior ceiling.
12. All James Hardi Materials shall be color plus "Dove Grey".
13. All fasteners shall be stainless steel.

14. All are non -egress windows.
15. Recoat structural metal connectors on 6 Pavilions to include: Hand -tool clean, grind to
remove loose rust scale. Spot prime bare metal with Amercoat 1 component Epoxy.
Apply 1 full coat of Amercoat Epoxy Apply 2 top coats of Durethane DTM Urethane
Mastic.
16. Prepare, concrete and steel substrates and stucco 40 concrete columns with color
entrained synthetic stucco.
17. Remove existing light fixtures on ceiling and replace with HALO H7RICAT remodel
recessed cans, Halo 30wat white baffle trims and LED R30 12 watt amber turtle friendly
lamps. 11 new cans total. 9 (see cut sheets attached)
18. Replace existing wall mount fixture with 1 Progress P5712-30 white cylindrical fixtures
with LED R30 12 watt amber turtle friendly lamp. (see cut sheet attached)
19. Deposit all demolition materials in dumpsters provided by City to be located in parking
lot in ADA spaces South of Pier to be temporarily fenced by City for laydown yard.
Add Alternate Scope of Work:
1. Demolition and removal of 1st level vinyl siding up to the soffit and patio ceiling level.
Supply and install Color Plus Dove Grey, Hardi-lap siding with 60 mil peel and stick
underlayment. Exterior grade plywood and window replacement. Hardi Wrap all wood
columns, trim windows, doors, corners and wall to soffit intersect with Hardi-Board
Trim. See detailed cost breakdown below.
Exclusions:
1. Concrete repairs.
2. Replacement of any structural connectors.
3. Replacement of structural framing, plywood or sheathing.
4. Plumbing or Mechanical Work.
5. Electrical work beyond what is described above.
6. Removal or replacement of low voltage mounted to building.
7. Termite or pest control.
8. Painting other than structural connectors on Pavilions.
9. Doors.
10. Temporary fencing at laydown yard in parking lot.
11. Permitting or permitting fees.
12. Materials testing.
13. Dumpsters or debris haul -off.

Project Duration shall be 90 calendar days from the date of the Notice to Proceed. Onsite
Construction base contract is estimated at 8 weeks, add 4 weeks for 1st floor alternate. We will
require periodic shut downs of baithouse.
Existing Contract: This proposal is submitted in conjunction with the existing Construction
Manager at Risk Services Continuing Contract entered into with the City of Clearwater on June
15, 2016, based on RFC) #24-16.
If GMP Exceeds $150,000: Per Section III, Article 5.1 of the Contract Specifications, the
contractor shall provide to the public entity a certified copy of the recorded bond. Once the City
receives a certified copy of the recorded bond, a Notice to Proceed may be issued. Include the
attached bond form as well as the Power of Attorney.
For work performed, invoices shall be submitted to the City of Clearwater, Engineering
Department, Attn: Veronica Josef, Senior Staff Assistant, P.O. Box 4748, Clearwater, Florida,
33758-4748. Contingency services may be billed only after written authorization is provided by
the City to proceed with those services.

BY THIS BOND, We Khors Construction, Inc. as Contractor, and FCCI Insurance Group Surety, a corporation, as Surety, are
bound to the City of Clearwater, Florida, herein called Owner, in the sum of $375,267.19 Three hundred, seventy-five
thousand, two hundred, sixty-seven and 19/100 dollars for payment of which we bind ourselves. our heirs, personal
representatives, successors, and assigns, jointly and severally.
THE CONDITION OF THIS BOND is that if Contractor:
1. Performs the proposal dated November 15, 2017, between Contractor and Owner for construction of PROJECT #KC -
1801 PIER 60 BAIT HOUSE AND 6 PAVILION RE ROOF the contract documents being made a part of this bond by reference
(which include the Advertisement for Bids, Proposal, Contract, Surety Bond, Instructions to Bidders, General Conditions, Plans,
Technical Specifications and Appendix, and such alterations as may be made in said Plans and Specifications as therein
provided for), at the times and in the manner prescribed in the contract; and
2. Promptly makes payments to all claimants, as defined in Section 255.05(1), Florida Statutes, supplying Contractor with
labor, materials, or supplies, used directly or indirectly by Contractor in the prosecution of the work provided for in the contract;
and
3. Pays Owner all losses, damages, expenses, costs, and attorney's fees, including appellate proceedings, that Owner
sustains because of a default by Contractor under the contract; and

4. To the limits of § 725.06(2), Florida Statutes, shall indemnify and hold harmless Owner, their officers and
employees, from liabilities, damages, losses and costs, including, but not limited to, reasonable attorney's fees, to the

utilized by Contractor in the performance of the construction contract; and

5. Performs the guarantee of all work and materials furnished under the contract for the time specified in the
contract, then this bond is void; otherwise it remains in full force.
6. Any action instituted by a claimant under this bond for payment must be in accordance with the notice and time
limitation provisions in Section 255.05(2), Florida Statutes.
7. Any changes in or under the contract documents and compliance or noncompliance with any formalities
connected with the contract or the changes does not affect Surety's obligation under this bond, and Surety does hereby
waive notice of any such change, extension of time, alteration or addition to the terms of the contract or to the work or to
the specifications.
