Trustee Caudell moved to approve the new hires for acceptance
into the Pension Plan as listed. The motion was duly seconded and
carried unanimously.
4.2 Approve the following request of employee Thomas Leonard, Marine and Aviation
Department, to vest their pension as provided by Section 2.419 of the Employees’
Pension Plan.
Thomas Leonard, Marine Facility Operator, Marine and Aviation Department,
was employed by the City on March 31, 2007, and began participating in the
Pension Plan on that date. Mr. Leonard terminated from City employment on
March 31, 2017.
The Employees’ Pension Plan provides that should an employee cease to be
an employee of the City of Clearwater or change status from full-time to
part-time after completing ten or more years of creditable service (pension
participation), such employee shall acquire a vested interest in the retirement
benefits. Vested pension payments commence on the first of the month
following the month in which the employee normally would have been eligible
for retirement.
Section 2.416 provides for normal retirement eligibility for non-hazardous duty
employees hired prior to the effective date of this reinstatement (January 1,
2013), a member shall be eligible for retirement following the earlier of the date
on which a participant has reached the age of fifty-five years and completed
twenty years of credited service; the date on which a participant has reached
age sixty-five years and completed ten years of credited service; or the date on
which a member has completed thirty years of service regardless of age. For
non-hazardous duty employees hired on or after the effective date of this
restatement, a member shall be eligible for retirement following the earlier of
the date on which a participant has reached the age of sixty years and
completed twenty-five years of credited service; or the date on which a
participant has reached the age of sixty-five years and completed ten years of
credited service. Mr. Leonard will meet the non-hazardous duty criteria and

Section 2.416 provides for normal retirement eligibility for hazardous duty
employees, a member shall be eligible for retirement following the earlier of the
date on which the participant has completed twenty years of credited service
regardless of age, or the date on which the participant has reached fifty-five
years and completed ten years of credited service.
Trustee Cundiff moved to approve the following request of
employee Thomas Leonard, Marine and Aviation Department, to
vest their pension as provided by Section 2.419 of the Employees’
Pension Plan. The motion was duly seconded and carried
unanimously.
4.3 Approve the following request of employees Cameron Hill, Fire Department, Benjamin
McBride, Police Department, Kelly O’Brien, Engineering Department and William
Wargin, Fire Department for a regular pension as provided by Sections 2.416 and
2.424 of the Employees’ Pension Plan.
Cameron Hill, Firefighter, Fire Department, was employed by the City on
October 14, 1996, and his pension service credit is effective on that date. His
pension will be effective April 1, 2017. Based on an average salary of
approximately $60,084.17 over the past five years, the formula for computing
regular pensions and Mr. Hill’s selection of the 100% Joint and Survivor
Annuity, this pension benefit will be approximately $32,880.12 annually.
Benjamin McBride, Police Lieutenant, Police Department, was employed by the
City on August 12, 1985, and his pension service credit is effective on that
date. His pension will be effective April 1, 2017. Based on an average salary
of approximately $111,052.80 over the past five years, the formula for
computing regular pensions and Mr. McBride’s selection of the 100% Joint and
Survivor Annuity, this pension benefit will be approximately $94,219.44
annually.
Kelly O’Brien, Engineering Specialist III, Engineering Department, was
employed by the City on September 23, 1985, and his pension service credit is
effective on April 22, 1986. His pension will be effective April 1, 2017. Based
on an average salary of approximately $74,274.82 over the past five years, the
formula for computing regular pensions and Mr. O’Brien’s selection of the Life
Annuity, this pension benefit will be approximately $63,200.16 annually.
William Wargin, Fire Lieutenant, Fire Department, was employed by the City
on March 3, 1997, and his pension service credit is effective on that date. His
pension will be effective April 1, 2017. Based on an average salary of

approximately $78,460.55 over the past five years, the formula for computing
regular pensions and Mr. Wargin’s selection of the Life Annuity, this pension
benefit will be approximately $43,442.64 annually.

employees hired prior to the effective date of this reinstatement (January 1,
2013), a member shall be eligible for retirement following the earlier of the date
on which a participant has reached the age of fifty-five years and completed

City on February 24, 2003, and began participating in the Pension Plan on that date. Mr.
Wells terminated from city employment on April 22, 2017.

annually.
Michael Cieslak, Police Officer, Police Department, was employed by the City on October 17,
1994, and his pension service credit is effective on that date. His pension will be effective May
1, 2017. Based on an average salary of approximately $82,780.81 over the past five years,
the formula for computing regular pensions and Mr. Cieslak’s selection of the 100% Joint and
Survivor Annuity, this pension benefit will be approximately $50,550.36 annually.
Jean Griggs, Police Records Manager, Police Department, was employed by the City on
October 2, 1995, and her pension service credit is effective on that date. Her pension will be
effective June 1, 2017. Based on an average salary of approximately $67,040.72 over the past
five years, the formula for computing regular pensions and Ms. Griggs’ selection of the Life
Annuity, this pension benefit will be approximately $39,939.96 annually.
Suzanne Kravik, Police Service Technician, Police Department, was employed by the City on
April 27, 1987, and her pension service credit is effective on that date. Her pension will be
effective May 1, 2017. Based on an average salary of approximately $51,549.31 over the past
five years, the formula for computing regular pensions and Ms. Kravik’s selection of the Joint
and Survivor Annuity, this pension benefit will be approximately $42,536.04 annually.
Mark Serio, Utilities Maintenance Supervisor I, Public Utilities Department, was employed by
the City on July 30, 1990, and his pension service credit is effective on June 17, 1991. His
pension will be effective May 1, 2017. Based on an average salary of approximately
$52,926.67 over the past five years, the formula for computing regular pensions and Mr .
Serio’s selection of the 66 2/3% Joint and Survivor Annuity, this pension benefit will be

oversee the investment of City of Clearwaters pension fund. Specifically to review fund managers
performance and other business as deemed necessary.

3. What background and /or qualifications do you have that you feel would qualify you to serve on
this Committee?
I have been employed in the Banking industry for the last 18 years, held several roles including fixed
income desk liaison working with Citi's biggest hedge funds. I am well versed in investment fund
terminology and extensive financial product knowledge.
4. Why do you want to serve on this Committee?
I have business and life skills I know could benefit our beautiful city of Clearwater and want to secure
this city's future for my children and their children and try to preserve its appeal by helping any way I
can.

I've worked at AmeriLife 20 years, mainly as Supervisor for Agent Contracting and Licensing Department. This
Includes supervision of 9 employees. Ordering and reviewing background reports of our Insurance agents and collecting necessary
documents from agents for review by Legal Department if needed. Act a s Iiason for General Managers and Insurance Agents —
providing guidance as needed to ensure compliance with contracting and licensing guidelines of AmeriLife and its insurance
carriers in the states we service.
4. Why do you want to serve on this Board?
To participate further and learn more about my community; and to support Clearwater as best I can.

Processed fingerprinting and background screening paperwork for appropriate agencies,
coordination of monthly manager meetings, assisted in development of personnel policies,
parent handbook and brochures as well as proposals for funding of expansion projects.
Also, worked with the Training Department — assisted development of computer software and
record keeping system for Florida State Child Care Working Training Program (1985 -88) and in-
service training for Title XX standards; production of newsletters, course announcements and
surveys; arranging monthly provider meetings, compiling agendas and taking minutes.
