Dear Mr. Chin:
I am pleased to submit the following proposal for the Construction Management of the
Morningside Recreation Center Project as listed above. This proposal is complete in accordance
with the Scope of Work provided by City of Clearwater and dated January 2, 2017, which includes:
for 16- 0035 -PR -A; technical specifications of pool work renovations provided by GB Collins
Engineering PA, plans by Griner Engineer, Inc. and Wannamaker Jensen Architects, Inc. And for
16- 00035 -PR -B Technical specification of demolition and other miscellaneous work as shown on
drawings developed by Deuel & Associates and the Report of Neshap Demolition Survey,
Sampling and Evaluation of Asbestos - Containing Materials for Eastern Building in Pool Complex
by Greenfield Environmental on April 5, 2016, and Existing Conditions Overall Survey provided
by Deuel Associates dated April 11, 2106. Wannemacher Jansen Architects, Inc. for Basketball
Court Retaining Wall January 9, 2017,
Specific Exclusions:
Permitting fees or fees paid to utility companies.
Dumpsters or debris haul -off.
Builder's Risk Insurance.
Sodding/Import Fill Materials.
Fencing temporary of permanent.
We propose to perform the aforementioned base scope for the sum of $564,095.08 five hundred
sixty -four thousand, ninety -five, and 08/100 dollars).

Project Duration shall be 75 calendar days from the date of the Notice to Proceed.
Existing Contract: This proposal is submitted in conjunction with the existing Construction
Manager at Risk Services Continuing Contract entered into with the City of Clearwater on June 15,
2016, based on RFQ #24 -16.
If GMP Exceeds $150,000: Per Section III, Article 5.1 of the Contract Specifications, the
contractor shall provide to the public entity a certified copy of the recorded bond. Once the City
receives a certified copy of the recorded bond, a Notice to Proceed may be issued. Include the
attached bond form as well as the Power of Attorney. (If GMP is less than $150,000 a bond is not

For work performed, invoices shall be submitted to the City of Clearwater, Engineering
Department, Attn: Veronica Josef, Senior Staff Assistant, P.O. Box 4748, Clearwater, Florida,
33758 -4748. Contingency services may be billed only after written authorization is provided by the
City to proceed with those services.
