Engineering Department, whose address is Post Office Box 4748, Clearwater, Florida 33758-4748, a
political subdivision of the State of Florida, party of the second part,
WITNESSETH
That the said party of the first part, for and in consideration of the sum of Ten Dollars
($10.00) and other good and valuable considerations to them in hand paid by the said party of the
second part, the receipt whereof is hereby acknowledged, does hereby grant and release unto the said
party of the second part, a perpetual sidewalk easement over and across the following described
property located in Pinellas County, Florida, to wit:
Lands described in legal description attached as Exhibit "A"hereto
and made a part hereof, hereinafter referred to as the "Easement".

referenced City of Clearwater project, desires to make a voluntary donation of
said property or property interest to the City of Clearwater for the use and benefit
of the project and the citizens of Clearwater.
The undersigned hereby acknowledges that he/she has been fully advised by the
City representative of his/her right to have the referenced property or property
interest appraised, to accompany the appraiser during the appraisal inspection of
the property, to receive full compensation for the above referenced property, and
to receive reimbursement for reasonable fees and costs incurred, if any. Having
been fully informed of the above rights, I hereby waive those rights unless
otherwise noted below.
