t/ The window sill reinforcing had not yet been installed at the uppermost level. Upon being advised,
the contractor commenced their installation. Otherwise, no exceptions were taken. The center piers
of the end wall are reinforced the same as the center was below on the lower levels as previously
reported.
o Checked reinforcing placement for the eastlwest infill wall abutting the center shear wall tower along the
outboard edge of the slab at all elevated floor levels.
t/ No exceptions were taken.
o Checked the installation work in progress for the veneer walls being installed abutting the columns on
the north balconies. This work commenced at the uppermost floor level.

[tj Checked reinforcing placement for the second lift of block masonry for the west half of the sixth floor
level excluding one bay at the southwest corner currently in use for material access.
t/ No exceptions were taken as the reinforcing was installed in accordance with the requirements of
the design plans.
t/ Okay to grout the noted sections of block masonry.
[tj Checked reinforcing placement for the vertical balance of the main roof parapet walls and the penthouse
walls.
K At the penthouse tie beams, the vertical bars had not been terminated with a hook, the stirrup
spacing had not been reduced to 6" o.c. over the door opening and embed plates for the roof truss
anchors were not being installed. The contractor indicated that epoxied truss anchors will be
installed at a later date and that the missing reinforcing bars will be installed.
K To achieve the top of parapet elevation, a thin cast-in-place concrete cap was typically being
installed of plain concrete. The contractor recognized the need for longitudinal reinforcing and
commenced the installation of 2-#5 continuous bars throughout their lengths.

components described in the Threshold Inspection Plan for this project complies with the
permitted documents and the specialty shoring design professional engineer has
ascertained that the shoring and reshoring conforms to the shoring and reshoring plans
submitted to the enforcement agency.

The purpose of this visit was to observe testing of the auger-cast piles conducted by "BTL Engineering
Service". Pile testing was observed and test result shall be submitted for verification by the test lab to the
engineer of record and special inspector.
Test required verifying the capacity of the test pile up to 160 ton. Observed test up to 70 ton and future
testing was to be conducted throughout the day to reach the 160-ton limit.

The purpose of this visit was to observe the contractor performing construction of auger cast piles.
· Observed size and spacing of reinforcing steel for shear piles. Ties were spaced 12" o.c. and the
specified distance is 8" o.c. Contractor was advised to correct spacing.
Because of the delay of concrete the auger cast pile installation for this day was cancelled and
rescheduled for the next day.
Based on our observations, all items appear to conform to the approved documents, with the noted
exceptions.

bar was observed to be 37 inches vs. required lap of 48 inches, Pile contractor identified specific
cage to be reworked by painting orange,
· Randomly check partially assembled cages in cage assembly area for bar size and tie spacing,
All check OK.

- the south corridor wall by the elevator lobby levels 1 to 5
- the trash room wall at the southeast corner at levels 1 to 5
- the trash chute room wall at the southwest corner at levels 1 to 4.
K At the 1st and 2nd levels at the south corridor walls by the elevator lobby, the reinforcing did not
have the correct splice length on the reinforcing steel. The contractor was made aware of this
Condition for correction.
ttI Okay to grout the above walls upon correction of the noted exception. The reinforcing is now in
accordance with the design drawing requirements.
ItI Checked reinforcing placement in the CM U piers on the 2nd level at the north, east and west sides of
the building. Also checked the piers on the south side of level 1 and 2.
ttI No exceptions were noted as the reinforcing was in accordance with the design documents.

Items Inspected:
o Checked the metal roof truss installation at the east side stair tower.
)t Some of the 5/8" dia. bolts epoxied into the concrete beam do not have enough setback from the
edge of the concrete.
)( All of the comer trusses need to have clips installed and fastened properly to the concrete beam.
)t At the west wall, one truss needs a shim installed under the truss.
o Checked the installation of the metal roof decking on the east side stair trusses.
)t No screws were installed on the side laps throughout the entire roof.
* We will return to the site for re-inspection after the required repairs have been made. The interior
side of the walls were marked in the field with the necessary corrective actions.
o Checked metal roof truss installation at the west side stair tower.
)t At the southwest comer, a clip is not properly tightened down. The wall was marked and the
contractor was made aware of this condition.
)t At the west wall, a clip to truss connection was marked as not being complete or tightened.
)t At the south wall, several roof trusses needed shims installed under them.
)t At the southeast comer, a clip to truss connection was missing all of the screws.
)t All of the comer trusses require a clip to be installed and fastened to the concrete beam.
III Checked the installation of the metal roof decking on the west stair tower trusses.
)t One sheet of decking (located on the southern wall just past the concrete wall) was bent and did not
have all of the screws installed into each truss.
* We will return to the site for re-inspection after the required repairs have been made.

)t At the west tower, shims still need to be placed under the trusses.
I?I Re-inspected the installation of the metal roof decking on the east and west stair towers.
t/ No exceptions were noted as the repairs were found to be acceptable.
t/ Okay to install roofing.
o Checked reinforcing placement in the block masonry break-away walls located on the ground floor level
at the west elevator and the trash room. The walls inspected at the west elevator were the east and
south walls. The walls inspected at the trash room were the north and south walls.
t/ No exceptions were taken as the reinforcing was in accordance with the design documents.
t/ Okay to grout the above noted walls.

Article 3-1508,
10. Construction or repairing of buildings. The erection including
excavation, demolition, alteration or repair of any building so as to
create a loud and raucous noise other than between the hours of
7:00 a.m. and 6:00 p.m. on any day or at any time on Sunday,
except in case of urgent necessity in the interest of public health
and safety and then only with a permit from the building official,
which permit may be granted for a period not to exceed three
working days or less while the emergency continues and which
permit may be renewed for successive periods ofthree days or less
while the emergency continues. If the building official should
determine that the public health and safety necessitates the
issuance of such a permit and will not be impaired by the erection,
demolition, alteration or repair of any building or the excavation of
streets and highways within the hours of 6:00 p.m. and 7:00 a.m. or
on Sunday, the building official may grant permission for such
work to be done within such hours or within a shorter time period
during such hours, upon application being made at the time the
permit for the work is issued or during the progress of the work.

west end of the building.
II' The bars were too close to the forms and required adjustments which were commenced in
our presence.
