Wharton- Smith, Inc. (WS!) is pleased to submit our proposal in response to the request to relocate the
sewer and water utility lines on S Belcher Rd and Gulf -to -Bay Blvd in accordance with the plans prepared
by the city of Clearwater engineering department dated 3/30/16 and the waterline drawing plan added on
9/27/16, for a Guaranteed Maximum Price of $696,868.70.
Scope of Work:
This proposal includes the work associated to off -set the water main on the northwest corner of Belcher
Rd, the replacement of 20' of sewer line at seven (7) locations on SR -60, off -set 210 If of 8" water main and
replace 20 If of 6" water main at northeast corner of Belcher Rd., and replace and relocate 215 If of 8"
sewer main.
The basis of our proposal shall include:
• All traffic maintenance and MOT barricades.
• Demolition of concrete and asphalt as necessary.
• Temporary removal of street light poles and rewiring to keep other lights operational at night.
• Demolition of existing pipe.
• Excavation, dewatering, shoring and trench protection, installation of new pipe and valves, reverse
deadman as necessary.
• Installation of service laterals are by others.
• Installation of line stops as necessary to complete work.
• Pressure testing, disinfection and bactees of the water lines.
• Soils testing to confirm compaction per FDOT requirements.
• Asphalt patches as required for work on the water main off -set on Belcher.
• Replacement of any sidewalk are by others.

• TV inspection of sewer after work is completed.
• As- builts of installed lines.
• Landscaping & Irrigation are to be installed by others.
• If contaminated or unsuitable soil is encountered during excavation. a negotiated fee will be
established to remove and replace this material.
• Signalization repair is provided by others.
• No grade profile on the extra waterline work, assume depths are at max 5'. Anything over 5' deep
will need to be renegotiate for extra time and dewatering.
• Permits are by others.
A detailed breakdown of the scope of work is provided in Attachment A.
Working adjacent to a state road 60, WSI will be required to manage the project activities as well as
coordination with FDOT and the FDOT contractor. WSI would provide a highly qualified team of
professionals to successfully manage this project which has a short and concentrated scope and schedule.
All work will be performed at night, during non -peak traffic hours. WSI will be attending all required
meetings with the FDOT contractor. We will be coordinating and scheduling work and work zone impacts
with the FDOT contractor and the city of Clearwater for the duration of the project. The logistics and the
timing of the work will be critical so that there is little impact to traffic patterns and the work being
performed by the FDOT contractor. WSI also intends to be part of any community meeting required to
discuss the project impacts with the community. WSI will coordinate and verify that all work is in
compliance with FDOT guidelines and regulations.
This proposal includes a $25,000 allowance for soil testing activities and $10,000 allowance for relocation
of street Tight poles. As these funds are an allowance, any funds that are not spent will be forfeited back to
the City of Clearwater.

Project Duration shall be _85_ calendar days from the date of the Notice to Proceed. A breakdown of
duration of each item is provided in attachment B. These line items will be performed concurrently so that
the project is completed within 85 calendar days.

CONSTRUCTION GROUP
Existing Contract: This proposal is submitted in conjunction with the existing Construction Manager at Risk
Services Continuing Contract entered into with the City of Clearwater on August 19, 2016, based on RFO
#24 -16.
If GMP Exceeds $150,000: Per Section III, Article 5.1 of the Contract Specifications, the contractor shall
provide to the public entity a certified copy of the recorded bond. Once the City receives a certified copy of
the recorded bond, a Notice to Proceed may be issued. Include the attached bond form as well as the Power
of Attorney.
For work performed, invoices shall be submitted to the City of Clearwater, Engineering Department, Attn:
Veronica Josef, Senior Staff Assistant, P.O. Box 4748, Clearwater, Florida, 33758 -4748. Contingency
services may be billed only after written authorization is provided by the City to proceed with those
services.
