Information Technology (IT) Consulting Services
This Amendment 14 ( "Amendment"), effective as of September 1, 2015, to the information Technology (IT)
Consulting Contract No. 973-561-1()-1 ('Contract) is effective between the State of Florida, Department of
Management Services ( "Department"), and Advanced Data Solutions, Inc. ( "Contractor"). Department a
Contractor are collectively referred to herein as the "Parties." nd
RECITALS
L The Contract was awarded to the Contractor on September 15, 2009 to oontinue through August 31,
2012, for the provision of Information Technology (IT) Consulting Services, pursuant to h No. 2-
973- 561 -K; and
II. The Contract was subsequently renewed until August 31, 2015; and
Ill. In accordance with Section 287.057(12), Florida Statutes, upon mutual agreement, the Department
and the Contractor may extend the Contract, for a period that may not exceed six (6) months; and
NOW THEREFORE, for the mutual covenants contained herein, the Parties agree as follows.
