The meeting was called to order at 9: 10 a.m. by Chairman Shuford in the DRC Conference
Room B.
MOTION: Lou Hilton moved to approve the minutes of the December 8, 1994 meeting as
submitted. The motion was seconded by Bob Maran and passed unanimously.
ITEM #1 Palm Garden of Clearwater; Located at the southwest corner of McMullen-Booth
Road and Curlew Road. The applicant proposes to construct a 5,200 sq. ft. Therapy Addition to
the existing Palm Garden facility. The Engineer is requesting comments from staff to facilitate
this improvement.
Pertinent comments from the Department representatives are incorporated into the motion listed
below.
MOTION: Lou Hilton moved to approve amended site plan subject to the following conditions:
Central Permitting
1. The requisite building permits must be procured within one (1) year from the date of
certification of the final site plan and all requisites certificates of occupancy must be
obtained within three (3) years of the date of certification of the site plan.

The certified site plan shall be amended prior to issuance of a building permit.
Environmental Management
1. Prior to issuance of a building permit, a landscape plan shall be submitted to
Environmental Management.
2. Prior to the issuance of a building permit, a clearing and grubbing permit and a tree
removal permit (or a no tree verification form) are required.
3. This site has previously used 25% allowable tree removal; consequently all trees
removed must be replaced on an inch for inch basis.
The motion was seconded by Bob Maran and carried unanimously.
ITEM #2 North Bay Community Church - Located at 3170 McMullen-Booth Road. The
applicant proposes to construct a paved 70 parking space lot with an expanded retention area for
stormwater.
Pertinent comments from the Department representatives are incorporated into the motion listed
below.
MOTION: Lou Hilton moved to approve amended site plan subject to the following conditions:
Central Permitting
1. Signs and fencing/walls are subject to separate review and permitting processes.
2. The requisite building permits must be procured within one (1) year from the date of
certificatiC?n of the final site plan and all requisites certificates of occupancy must be
obtained within three (3) years of the date of certification of the site plan.
Fire Department
1. The minimum turning radius for designated drives and fire lanes shall be 42 feet.
Traffic Engineering
1. Site plan shall indicate handicapped spaces.
Environmental Management
1. Prior to issuance of a building permit, a landscape plan shall be submitted to
Environmental Management.
2. Prior to the issuance of a building permit, a clearing and grubbing permit and a tree
removal permit (or a no tree verification form) are required.
3. Prior to certification, a copy of the SWFWMD permit application and calculations shall

submitted to Environmental Management.
The motion was seconded by Bob Maran and carried unanimously.
ITEM #3 - Bayside Nursing Pavilion; Located at the western portion of the existing Bayview
Gardens Continuing Care Retirement Community. The Commission received and referred a
similar project at this location on June 16, 1994. The applicant has modified the initial site plan
by relocating the proposed two story 120 bed nursing home facility and a sales center with
associated required parking located away from the 100 year floodplain, placing the facility at the
western portion of the existing Bayview Gardens property. The applicant proposes to expand the
existing Bayview Gardens Continuing Care Retirement Community by constructing a 49,670 sq.
ft., two story, 120 bed nursing facility with associated parking and stormwater management. All
property owned by the applicant is now shown on the site plan.
MOTION: Lou Hilton moved to continue this item to January 19, 1995. The motion was duly
seconded and carried unanimously.
There being no further business before it, the meeting adjourned at 11 :53 a.m.
