Dear Ms. Hinson:
In response to your letter dated June 23, 2014, by mutual agreement and on behalf of the City of
Clearwater, I would like to extend the Agreement with Arts 4 Life Academy, Inc. for a period of five (5)
years commencing on the 1st day of October, 2013 and continuing through the 30th day of September,
2018 as per Article 1.2 of the Agreement. This five year extension is subject to the same terms and
conditions provided in the Agreement dated July 22, 2008.
Since mutual agreement is required, please sign and return the below as soon as possible.
