1. APPLICATION SUBMITTED (These tasks must be completed within one day after application deadline.)
Sets up file for application - Includes writing owners name, address & case number on outside cover of file and
then listing the scheduled meeting dates for DRC, CDB, CC, PPC, CPA, DCA (as applicable)
Requests legal descriptions to be reviewed by Engineering (to be returned corrected/approved by the following
Friday. The final approved Legal must contain Tom Mahony's initials)
Requests maps from Engineering (to be returned by the following Friday)(in an electronic format - see
procedures to be developed by Marc - see template)
Copies 400' scale zoning atlas page highlighting each case to be included in DRC package for the Legal
Department. Highlight DRC agenda of cases so Legal knows which one needs ordinances. Make note of any
cases with multiple property owners.
2. COMPLETENESS REVIEW
Confirms completeness, confirms DRC date and sends letter notifying applicant or applicant's representative of
the DRC, CDB, CC (also PPC and CPA if Land use Plan Amendment)
Begins review and discusses with any appropriate DRC members
Determine any unusual issues for review and any special assistance needed by DRC members (Le. traffic
analysis, environmental issues, etc.)
Conducts Site Visit and photographs site
Adds photographs to computer file (Permit Plan)
Notifies Pinellas County & PPC (within 5 days of receiving application) - for ANX only. Also e-mail of ANX
information to Police Dept. for determination they can serve)
Hand delivers approved legal descriptions to Legal (by. . . . . .)
Review maps from Engineering and immediately advise Engineering of any necessary corrections. E-mail
finalized maps to Legal (2" x 2", future land use, zoning and annexation maps)
Decision re sufficient or insufficient must be made prior to DRC meeting and discussed with Long Range Planning
Manager
3. DRC AGENDA
o Determines with LR Manager which applicants need to attend DRC (typically anx do not need to attend)
4. DRC CASE REVIEW MEETING & STAFF REPORT
Prepares draft staff report for Case Review Meeting and Long Range Planning Manager. Discusses any major
issues/problems with Long Range Planning Manager prior to case review.
Brings digital photos of site to Case Review Meeting
Adds comments to the draft of DRC Action Agenda on the shared drive to include all conditions and timing of
when issues must be resolved (This includes comments from other departments.) (whoever is running DRC
meeting)
Contacts applicant about major issues regarding case - prior to DRC meeting
Obtains draft ordinances from the Legal Dept. no later than the DRC date. Review for correctness. Long Range
Planning Manager must initial approval of ordinance before returning to the Legal Dept. Once Legal receives
approved legal description from Engineering and approved ordinance from Planning, the Legal Dept. shall issue
the final ordinances.
5. DRC MEETING
~ Discusses case issues
Outlines future steps of the Case.
Requests 15 copies of application.
. Give applicant public notice sign and get applicant to sign public notice posting form. If applicant does not attend,
make arrangements for applicant to pick up signs and sign notice forms).
6. COB AGENDA AND APPLICATIONS SENT TO CITY CLERK
~ Reviews resubmitted information within 7 days from applicants from DRC meeting and determines if sufficient and
ready to be on CDB agenda.
.. If case is sufficient, provide administrative analyst with information for clerk - 1 copy of the complete application
and attachments, 2" x 2" map, 400' scale zoning atlas with site highlighted. (Legal provides the ord. to the Clerk)
Adds application to CDB agenda
7. STAFF REPORT
O. Finalizes staff report for Long Range Planning Manager approval as applicable

Submit to DCA within 10 days of approval by the Countywide Planning Authority. ALL CORRESPONDENCE
TO DCA MUST BE REVIEWED BY THE LONG RANGE PLANNING MANAGER AND SIGNED BY THE
PLANNING DIRECTOR. Submit Small Scale Amendment Package - Transmittal Letter, Small Scale
Development Amendment form, case cover sheet, location map, adopted Future Land Use Plan Map, signed
ordinance, and public hearing notice.
File submission in small scale amendment file and Small Scale Development Amendment form in case file.
File approval/acceptance letter from DCA in case file and small scale amendment file
Small Scale package must also be copied to Tampa Bay Regional Planning Council

Proposed LarQe Scale Amendments
Prepare submission packages, to include: one transmittal letter to DCA outlining all cases included in
submission and 3 copies of the entire package. Transmittal letter must follow requirements of 9J-11.006 - see
files from 2003 and 2004. Package shall include copy of transmittal letter to other agencies, case cover sheet,
staff report, all maps (except annexation and rezoning maps), existing future land use map and proposed
future land use map, site photos, public notices, Future Land Use Plan amendment ordinance signed by
Legal, sign-in courtesy sheet, minutes from COB, minutes from 151 reading of City Commission.
Generally request preliminary review unless extremely complicated case. Make this decision with Long
Range manager.
After receive ORC report from DCA, schedule case for 2nd reading if no objections are raised. If objections are
raised, work with DCA to resolve. If denied, applicant should withdraw case or Commission should deny on
2nd reading.
Work with Long Range Planning Manager to notify City Clerk to schedule land use plan amendment for 2nd
reading. Second reading must occur within 120 days of receipt of ORC report. After 2nd reading, adopted
amendment must be submitted to DCA within 10 days. Adopted plan amendment submission package and 3
copies shall include: transmittal letter in accordance with 9J-11, transmittal letter to agencies, (one copy to
each agency), case cover sheet, adopted future land use plan map (this is new map), ~igned ordinance,
citizen courtesy sign-in sheet.

Dear Mr. Awerbach:
The Planning Department has reviewed your application for annexation (ANX2004-11016) for
the site at 1481 Sandy Lane. After preliminary review, staff has determined that the application
is incomplete; the following items must be submitted in order to complete your application and
begin staff review for Development Review Committee (DRC) consideration":
. Plat of survey- one (1) signed and sealed survey and fourteen (14) copies.
Please retrieve all items that were submitted to date and resubmit as part of a complete
application packet. I will discard all documents associated with the incomplete application on
December 10, 2004.
