1. Accept the employees listed below into membership in the City of Clearwater's
Employees' Pension Plan.
2. Eric Fetrow, Fire Department; Edward Bossom, Fire Department; Freddie Dixon, Parks
& Recreation Department; Terry Finch, Engineering Department; Paul Cheney,
Engineering Department; and Offie Cobb, Solid Waste Department, be granted regular
pensions under Section(s) 2.393 and 2.397 of the Employees' Pension Plan as
approved by the Pension Advisory Committee.
3. Carolyn Brink, City Manager's Department, be allowed to vest her pension under
Section(s) 2.397 and 2.398 of the Employees' Pension Plan as approved by the
Pension Advisory Committee.

Summary:
Teresa Finch, Environmental Manager, Engineering Department, was employed by the City on
November 19, 1984, and her pension service credit is effective on that date. Her pension will be
effective December 1, 2005.
Based on an average salary of approximately $65,658 per year over the past five years, the
formula for computing regular pensions, and Ms. Finch's selection of the Joint & Survivor
Annuity, this pension will approximate $37,962 annually.
Paul Cheney, Survey Party Chief, Engineering Department, was employed by the City on
September 5, 1978, and his pension service credit is effective on that date. His pension will be
effective December 1, 2005.
Based on an average salary of approximately $43,586 per year over the past five years, the
formula for computing regular pensions, and Mr. Cheney's selection of the Joint & Survivor
Annuity, this pension will approximate $32,579 annually.
Freddie Dixon, Recreation Programmer, Parks & Recreation Department, was employed by the
City on February 25, 1970, and his pension service credit is effective on June 3, 1978. His
pension will be effective December 1, 2005.
Based on an average salary of approximately $38,815 per year over the past five years, the
formula for computing regular pensions, and Mr. Dixon's selection of the 10-year Certain and
Life Annuity, this pension will approximate $31,625 annually.
Offie Cobb, Solid Waste Equipment Operator, Solid Waste Department, was employed by the
City on January 2, 1979, and his pension service credit is effective on that date. His pension will
be effective January 2, 2006.

Based on an average salary of approximately $40,708 per year over the past five years, the formula for
computing regular pensions, and Mr. Cobb's selection of the Joint & Survivor Annuity, this pension will
approximate $30,223 annually.
Eric Fetrow, Firefighter/Driver Operator, Fire Department, was employed by the City on August 22, 1983, and
his pension service credit is effective on that date. His pension will be effective November 1, 2005.
Based on an average salary of approximately $51,075 per year over the past five years, the formula for
computing regular pensions, and Mr. Fetrow's selection of the 100% Joint & Survivor Annuity, this pension will
approximate $30,796 annually.
Edward Bossom, Fire Lieutenant, Fire Department, was employed by the City on April 25, 1970, and his
pension service credit is effective on that date. His pension will be effective December 1, 2005.
Based on an average salary of approximately $74,225 per year over the past five years, the formula for
computing regular pensions, and Mr. Bossom's selection of the 100% Joint & Survivor Annuity, this pension will
approximate $72,135 annually.
These pensions were approved by the Pension Advisory Committee on December 8, 2005. Section 2.393 (p)
provides for normal retirement eligibility when a participant has reached age 55 and completed twenty years of
credited service, has completed thirty years of credited service, or has reached age 65 and completed ten years
of credited service. Section 2.393 also provides for normal retirement eligibility when a participant has completed
twenty years of credited service in a type of employment described as "hazardous duty" and further defines
service as a Firefighter Driver/Operator and Fire Lieutenant as meeting the hazardous duty criteria. Ms. Finch,
Mr. Dixon, Mr. Cheney, and Mr. Cobb qualify under the 20 years of service and age 55 criteria. Mr. Fetrow and
Mr. Bossom qualify under the hazardous duty criteria.

Carolyn Brink, City Manager's Department, be allowed to vest her pension under Section(s)
2.397 and 2.398 of the Employees' Pension Plan as approved by the Pension Advisory
Committee.
Summary:
Carolyn Brink, Executive Assistant, City Manager Department, was employed by the City on
February 7, 1994, and began participating in the Pension Plan on that date. Ms. Brink
terminated from City employment on November 18, 2005.
The Employees' Pension Plan provides that should an employee cease to be an employee of the
City of Clearwater after completing ten or more years of creditable service (pension
participation), such employee shall acquire a vested interest in the retirement benefits. Vested
pension payments commence on the first of the month following the month in which the
employee normally would have been eligible for retirement.
Section 2.393 (p) provides for normal retirement eligibility when a participant has reached age
55 and completed twenty years of credited service, has completed 30 years of credited service,
or has reached age 65 and completed ten years of credited service. Ms. Brink would have
completed at least 10 years of service and reached age 65 on October 27, 2006. Her pension
will be effective November 1, 2006. This pension was approved by the Pension Advisory
Committee on December 8, 2005.
