1. Accept the employees listed below into membership in the City of Clearwater's
Employees' Pension Plan
2. Joseph Zagone, Police Department, and Samuel Willi, Fire Department, be granted
regular pensions under Section(s) 2.393 and 2.397 of the Employees' Pension Plan as
approved by the Pension Advisory Committee.
3. James D. Johnson, Jr., Public Utilities Department; David D. Dornbusch, Solid Waste
Department; Lucette M Wombacher, Budget Department; and Roy J. Heinz, Police
Department, be allowed to vest their pensions under Section(s) 2.397 and 2.398 of the
Employees' Pension Plan as approved by the Pension Advisory Committee.

Joseph Zagone, Police Department, and Samuel Willi, Fire Department, be granted regular
pensions under Section(s) 2.393 and 2.397 of the Employees' Pension Plan as approved by the
Pension Advisory Committee.
Summary:
Joseph Zagone, Police Sergeant, Police Department, was employed by the City on August 12,
1985, and his pension service credit is effective on that date. His pension will be effective
September 1, 2005.
Based on an average salary of approximately $66,493 per year over the past five years, the
formula for computing regular pensions, and Mr. Zagone's selection of the 100% Joint &
Survivor Annuity, this pension will approximate $36,104 annually.
Samuel Willi, Fire District Chief, Fire Department, was employed by the City on January 15,
1973, and his pension service credit is effective on that date. His pension will be effective
September 1, 2005.
Based on an average salary of approximately $82,133 per year over the past five years, the
formula for computing regular pensions, and Mr. Willi's selection of the 100% Joint & Survivor
Annuity, this pension will approximate $72,468 annually.
These pensions were approved by the Pension Advisory Committee on September 23, 2005.
Section 2.393 provides for normal retirement eligibility when a participant has completed twenty
years of credited service in a type of employment described as "hazardous duty" and further
defines service as a Police Sergeant and Fire District Chief as meeting the hazardous duty
criteria. Mr. Zagone and Mr. Willi qualify under the hazardous duty criteria.

Summary:
James D. Johnson, Jr., Utilities Mechanic, Public Utilities Department, was employed by the City
on March 20, 1995, and began participating in the Pension Plan on that date. Mr. Johnson
terminated from City employment on August 5, 2005.
Roy J. Heinz, Police Officer, Police Department, was employed by the City on February 26, 1995,
and began participating in the Pension Plan on that date. Mr. Heinz terminated from City
employment on August 17, 2005.
David D. .Dornbusch, Solid Waste Worker, Solid Waste Department, was employed by the City
on April 24, 1989, and began participating in the Pension Plan on that date. Mr. Dornbusch
terminated from City employment on August 26, 2005.
Lucette M. Wombacher, Senior Accountant, Budget Department, was employed by the City on
December 27, 1993, and began participating in the Pension Plan on that date. Ms. Wombacher
terminated from City employment on September 30, 2005.
The Employees' Pension Plan provides that should an employee cease to be an employee of the
City of Clearwater after completing ten or more years of creditable service (pension
participation), such employee shall acquire a vested interest in the retirement benefits. Vested
pension payments commence on the first of the month following the month in which the
employee normally would have been eligible for retirement.
Section 2.393 (p) provides for normal retirement eligibility when a participant has reached age
55 and completed twenty years of credited service, has completed 30 years of credited service,
or has reached age 65 and completed ten years of credited service. Section 2.393 also (p)
provides for normal retirement eligibility when a participant has completed twenty years of
credited service in a type of employment described as "hazardous duty" and further specifically
defines service as a Police Officer as meeting the hazardous duty criteria.

Mr. Johnson would have completed at least 10 years of service and reached age 65 on January
4, 2009. His pension will be effective February 1, 2009. Mr. Dornbusch would have completed
at least 20 years of service and reached age 55 on April 24,2009. His pension will be effective
May 1, 2009. Ms. Wombacher would have completed at least 10 years of service and reached
age 65 on March 14, 2013. Her pension will be effective April 1, 2013. Mr. Heinz would have
completed 20 years of hazardous duty on February 26, 2015. His pension will be effective
March 1, 2015. These pensions were approved by the Pension Advisory Committee on
September 23, 2005.
