Dear Mr. Dwyer:
I received a request from your father Tom Dwyer requesting that the appropriate paperwork be sent to you so that
your properties can be connected to sanitary sewer when it is available in your neighborhood. Records indicate
that you own four properties in this area:
1. 1928 Macomber Ave
2. 1932 N Betty Lane
3. 1942 N Betty Lane
4. 1996 N Betty Lane
To date, we have recorded Agreements to Annex for the properties at 1928 Macomber Ave and 1932 N Betty
Lane. There is no other paperwork required prior to paying the required fee (if it hasn't already been paid) in
order to connect. The properties at 1942 and 1996 N Betty Lane are eligible to annex because they are adjacent to
properties already located within the City; therefore the next step would be to complete the enclosed Petition for
Annexation and other paperwork before payment of the $900 impact fee (per unit) can be made. These documents
must be signed by all owners listed on the deed, and signatures must be notarized. Once this is complete, the
documents can be returned to me and we will proceed with annexation.
In order to assist, I have completed certain fields on the documents about which we are typically asked. Please
add your contact information on pages 1 and 5. On page 2, please indicate if either of these properties is a rental
property, and please confirm that they are 1-unit residences (defined as having only one kitchen). Page 5 must be
completed with information related to any option to purchase the property; please review this and complete it in
its entirety. Page 6 is only required if you want to authorize someone else to serve as your representative/agent.
