CONTRACT AGREEMENT City of Clearwater Parks and Recreation Contract Agreement AMENDMENT The Agreement between Coordinated Child Care of Pinellas, Inc. (CCC) and City of Clearwater Parks and Recreation ("Agreement") fully executed as of October 1, 2013, is hereby amended by adding the following subsection to III. REIMBURSEMENT PROCESSES, paragraph A.: ADD a. The scholarship reimbursement per diem rate will be $22.56 per attended day during the remainder of the agreement period, effective January 1, 2014 — September 30, 2014. Only days attended by eligible youth receiving scholarship will be reimbursed. REMOVE b. Current Attachment 8 entitled, YOUTH SERVICES CLOSURE SCHEDULE FY 2013-2014, and replace with an updated 2014 Updated Closure Schedule (enclosed) which outlines days agency sites will remain closed through the end of the fiscal year. These days will be unpaid, as JWB reimbursement will be for each day an eligible OST youth participates. JWB will not reimburse for days a youth is absent or for holidays when the site is closed. Agreement is to be further amended by adding the following to existing ATTACHMENT 3 — PERFORMANCE MEASURES 2013-2014: c. Performance Measures are added to replace the current statement that reads: Performance Measures will be developed and this Agreement will be amended by December 31, 2013. ADD 1. Each provider site that has opted to participate in i-Ready have regularly attending JWB funded youth (attending 30 program days or more) participate in the i-Ready web-based assessment and curriculum. This is demonstrated by i-Ready Performance by School and Grade Report and OST attendance. (Baseline). #of regularly attending participants who completed i-Ready diagnostic and utilized online curriculum/#of regularly attending participants enrolled at participating site = rate (reported by site). 2. Regularly attending JWB funded youth (attending 30 program days or more) will remain in the OST program 180 calendar days or more as demonstrated by GEMS annual attendance reports. This will be measured by each site. (Baseline). #of regularly attending participants who were active for 180 calendar days/#of regularly attending participants= rate (reported by site). 3. 70% of regularly attending JWB funded youth (attending 30 program days or more) will attend OST programming a minimum of 3 days per week as measured by JWB researcher. (Baseline). #of regularly attending participants who attended a minimum of 3 days per week/#of regularly attending participants= rate (reported by site). 4. 100% of OST funded program sites will maintain a Quality Matrix Level Four, as documented in CCC site visit reports. (As reported in GEMS). 5. 100% of regularly participating OST participants who qualify (5t" Grade and above) will complete the OST pre and post survey during the administration windows. (Baseline). #of participants who took survey/#of active participants in grade 5 or above during survey administration window= rate (reported by site). All other terms and conditions of the Agreement shall remain in full force and effect. UPDATED OST CLOSURE SCHEDULE January 1, 2014 — September 30, 2014 DIRECTIONS: Although holidays will no longer be payable through OST, Program Providers are responsible for submitting their selected holidays and other closed dates for the remainder of the fiscal year when the Agreement Amendment is fully executed. Please type or print below the dates that your site(s) will be closed between January 1 and September 30, 2014. Please list them in order; type of holiday, or observed day (i.e. Veterans Day, In-Service, etc.) and exact date (month, day) of closure. These days will be non- reimbursable.
