Dear Mr. & Mrs. Swift,
I am enclosing the affidavit we discussed which needs to be signed by you both where indicated. You will
also need one other witness and a Notary at the time of signing. The City needs to receive the completed affidavit
before we can mark the space. We also talked about the need for a licensed funeral director to request the staking and
you indicated you would talk to Moss Funeral Home to make those arrangements. I am sure they would be honored to
continue to provide their services to your family.
Am also sending a copy of the yellow pages for this area that pertains to Monument Dealers. I have not heard
anything negative about any of them. Also enclosed is a page from the ordinance that governs the ceme~..;t @ will
note nothing is addressed to Block 6. That means those types of monuments mentioned in para (a~f page 4 can be
placed on the spaces in your family's lot. If you have any questions, please call me at 813-462-6684.
