TRUSTEES OF THE EMPLOYEES' PENSION FUND MEETING MINUTES

The Chair called the meeting to order at 9:00 a.m. at City Hall.
To provide continuity for research, items are in agenda order although not
necessarily discussed in that order.
Approval of Minutes -
09-12-2005
Trustee Petersen moved to approve the minutes of the September 12, 2005, meeting
motion
as submitted in written summation by the City Clerk. The was duly seconded and
carried
unanimously.
1. Accept the employees listed into membership in the City of Clearwater's Employees'
Pension Plan: Brandon Jerrido, April Eversole, Cky Ready, Kyle Wilson, Kathleen Troiani,
Naomi Malone, Micky Manlove, John Hennessy, Arthur Strick, Bradley Serata, Philip Palestra,
Bret Goodson, Leonard Rickard, Mel Melendez, Sherry Walker, Olin Crouch, Heather Latham,
Willie Hill, Debra Englander, Kelly Rogers, and David Sexsmith.
The Pension Advisory Committee recommended these employees be accepted into
membership in the City of Clearwater's Employees' Pension Plan.
Trustee Hamilton moved to accept the recommendation of the Pension Advisory
motioncarried
Committee. The was duly seconded and unanimously.
2. Joseph Zagone, Police Department, and Samuel Willi, Fire Department, be granted regular
pensions under Section(s) 2.393 and 2.397 of the Employees’ Pension Plan as approved by
the Pension Advisory Committee.
Joseph Zagone, Police Sergeant, Police Department, was employed by the City on
August 12, 1985, and his pension service credit is effective on that date. His pension will be

effective September 1, 2005. Based on an average salary of approximately $66,493 per year
over the past five years, the formula for computing regular pensions, and Mr. Zagone’s
selection of the 100% Joint & Survivor Annuity, this pension will approximate $36,104 annually.
Samuel Willi, Fire District Chief, Fire Department, was employed by the City on January
15, 1973, and his pension service credit is effective on that date. His pension will be effective
September 1, 2005. Based on an average salary of approximately $82,133 per year over the
past five years, the formula for computing regular pensions, and Mr. Willi’s selection of the
100% Joint & Survivor Annuity, this pension will approximate $72,468 annually.
These pensions were approved by the Pension Advisory Committee on September 23,
2005. Section 2.393 provides for normal retirement eligibility when a participant has
completed twenty years of credited service in a type of employment described as “hazardous
duty” and further defines service as a Police Sergeant and Fire District Chief as meeting the
hazardous duty criteria. Mr. Zagone and Mr. Willi qualify under the hazardous duty criteria.
Trustee Jonson moved that Joseph Zagone, Police Department and Samuel Willi, Fire
Department, be granted regular pensions under Sections 2.393 and 2.397 of the Employees’
motion
Pension Plan as approved by the Pension Advisory Committee. The was duly
carried
seconded and unanimously.
3. James D. Johnson, Jr., Public Utilities Department; David D. Dornbusch, Solid Waste
Department; Lucette M Wombacher, Budget Department; and Roy J. Heinz, Police
Department, be allowed to vest their pensions under Section(s) 2.397 and 2.398 of the
Employees’ Pension Plan as approved by the Pension Advisory Committee.
James D. Johnson, Jr., Utilities Mechanic, Public Utilities Department, was employed
by the City on March 20, 1995, and began participating in the Pension Plan on that date. Mr.
Johnson terminated from City employment on August 5, 2005.
Roy J. Heinz, Police Officer, Police Department, was employed by the City on February
26, 1995, and began participating in the Pension Plan on that date. Mr. Heinz terminated from
City employment on August 17, 2005.
David D. Dornbusch, Solid Waste Worker, Solid Waste Department, was employed by
the City on April 24, 1989, and began participating in the Pension Plan on that date. Mr.
Dornbusch terminated from City employment on August 26, 2005.
Lucette M. Wombacher, Senior Accountant, Budget Department, was employed by the
City on December 27, 1993, and began participating in the Pension Plan on that date. Ms.
Wombacher terminated from City employment on September 30, 2005.
The Employees’ Pension Plan provides that should an employee cease to be an
employee of the City of Clearwater after completing ten or more years of creditable service
(pension participation), such employee shall acquire a vested interest in the retirement
benefits. Vested pension payments commence on the first of the month following the month in
which the employee normally would have been eligible for retirement.

Section 2.393 (p) provides for normal retirement eligibility when a participant has
reached age 55 and completed twenty years of credited service, has completed 30 years of
credited service, or has reached age 65 and completed ten years of credited service. Section
2.393 also (p) provides for normal retirement eligibility when a participant has completed twenty
years of credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Police Officer as meeting the hazardous duty criteria. Mr.
Johnson would have completed at least 10 years of service and reached age 65 on January 4,
2009. His pension will be I'!ffl'!t::tive February 1, 2009. Mr. Dornbusch would have completed at
least 20 years of service and reached age 55 on April 24, 2009. His pension will be effective
May 1, 2009. Ms. Wombacher would have completed at least 10 years of service and reached
age 65 on March 14, 2013. Her pension will be effective April 1, 2013. Mr. Heinz would have
completed 20 years of hazardous duty on February 26, 2015. His pension will be effective
March 1, 2015. These pensions were approved by the Pension Advisory Committee on
September 23,2005.
Trustee Petersen moved that James D. Johnson, Jr., Public Utilities Department; David
D. Dornbusch, Solid Waste Department; Lucette M. Wombacher, Budget Department; and Roy
J. Heinz, Police Department, be allowed to vest their pensions under Sections 2.397 and 2.398
of the Employees' Pension Plan as approved by the Pension Advisory Committee. The motion
was duly seconded and carried unanimously.
