Rate Agreement
AMENDMENT
The Agreement between Coordinated Child Care of Pinellas, Inc. (CCC) and the City of
Clearwater ("Agreement") fully executed as of October 1, 2012, is hereby amended by
changing the end date of the agreement to be May 31, 2013.
All other terms and conditions of the Agreement shall remain in full force and effect.
IN WITNESS THEREOF, the parties have caused this amendment to be executed by their
undersigned officials as duly authorized.
