CONTRACT AMENDMENT
The Agreement between Coordinated Child Care of Pinellas, Inc. (CCC) and City of Clearwater
("Agreement") fully executed as of October 1, 2010, is hereby amended by adding the following
subsection to IV. FUNDS
ADD
B. CCC agrees to reimburse Provider at the daily attendance rate of 13.26 per day for up to
but not to exceed 52 care days for a maximum allocation of 20 Additional OST Summer
program scholarship slots totaling a maximum earning potential of up to but not to exceed
- 3.9790.40 camm_ _and ending August 19.2.011-for services rendered_
?? ~? pursuant to this Agreement and in accordance with program attendance and eligibility
requirements.
All other terms and conditions of the Agreement shall remain in full force and effect.
IN WITNESS THEREOF, the parties have caused this amendment to be executed by their
