applies for "Pyrofax" Gas Service upon the following terms and conditions:
L COMPANY AGREES:
(a) To sell and deliver to Customer, upon Customer's order,
in Company's cylinders conforming to Interstate Commerce
Commission's specifications, lent for that purpose, liquefied
petroleum gas sold under Company's trade-mark "Pyrofax",
in quantities sufficient to meet Customer's requirements for
such gas for use at the Installation Address during the period
covered by this agreement.
(b) To lend to Customer during the period of this agree-
ment "Pyrofax" gas regulating equipment listed as standard
by Underwriters' Laboratories, Inc.
(c) To lend upon Customer's request regulating equipment
for the replacement of unserviceable regulating equipment
and to make proper adjustment for the value of any gas lost

through any failure of regulating equipment or cylinders c.J

ll. CUSTOMER AGREES:
(a) Subj ect to Customer's right to terminate this agreemcnlt
in accordance with the provisions of Paragraph III. (a), to buy,'
from Company all of Customer's requirements of liquefied!
petroleum gas for use at the Installation Address during the
period covered by this agreement, and to pay the purchase
price thereof on demand of Company at or after delivery.
(b) To return each cylinder to Company at the Installation
Address when empty.
(c) To advise Company promptly if the regulating eqUIp-
ment shall fail to function properly.
(d) Upon termination of this agreement for any reason
whatsoever, to return promptly to Company all its regulating
equipment and cylinders in Customer's possession.
ID. BOTH PARTIES AGREE:
(a) This agreement shall continue until terminated by either party in the manner and upon the conditions following:
By Customer: At any time upon at least thirty days' written notice to Company.
By Company: At or after the expiration of three years from the date hereof upon at least thirty days' written notice to
Customer; and at any time upon written notice to Customer, upon failure of Customer to fulfill or conform to any of the
terms or conditions hereof, or if Customer shall sell or vacate the premises described as the Installation Address, or dis-
continue the use of "Pyrofax" gas at such Installation Address, or upon the filing of a petition in bankruptcy by or against.
or the appointment of a receiver for, or the insolvency or death of Customer.
(b) The price of "Pyrofax" gas and the terms of payment shall be in accordance with Company's schedule of prices in effect
at the time of delivery in the area in which the Installation Address is located. In addition, Customer shall pay Company the
amount of any excise taxes which Company may be required to pay with respect to sales or deliveries hereunder, except where
the law otherwise provides. Company may modify the purchase price and terms from time to time. In its schedule of prices,
Company may provide for reasonable demurrage charges on cylinders and/or a reasonable minimum price or charge per
month or per year for "Pyrofax" Gas Service.
(c) This agreement cannot be -changed or arty of its provisions varied except in writing signed by an authorized repre-
sentative of Company at its New York, N. Y., office. This instrument shall constitute a binding agreement between the
parties hereto only upon Company's written acceptance of this application in the space below by an authorized representative
of the Company at its New York, N. Y., office.

CONDITIONS
1. All regulating equipment and cylinders lent hereunder
by Company shall at all times remain the property of
Company.
2. Customer shall cause regulating equipment to be in-
BtaJIed at Customer's expense by a "Pyrofax" gas distributor
in accordance with the Standards of the National Fire Pro-
tection Association (Pamphlet 5 8) under a separate agree-
ment between Customer and distributor. Company does not
install regulating equipment.
3. Customer will obtain and familiarize himself with Com-
pany's printed instructions before using gas or regulating
equipment.
<L Company recommends that Customer use with "Pyrofax"
gas only those appliances which conform to the approval
requirements of the American Gas Associaiion Laboratories
for liquefied petroleum gas-burning appliances, but Com-
pany assumes no responsibility- whatsoever for any appli-
ances. Company does not sell, install or repair appliances.
5. Customer acknowledges that all regulating equipment
and cylinders and the gas content thereof, while at the In-
stallation Address, are within Customer's possession, custody
and control and are not withiTl Company's possession, custody
or coMrol except while being delivered or removed by Com-
pany. Customer shall be responsible for any loss or damage
(unless caused by act of God) of or to Company's equip-
ment or cylinders after receipt thereof by Customer and
until return thereof to Company. In the event of total loss
or destruction, Customer shall pay Company as liquidated

damages Thirty-five Dollars ($35) for each such regula..
equipment and Twenty-five Dollars ($25) for each s~
cylinder.
6. Customer shall not remove Company's regulating equip-
ment or cylinders from the Installation Address. Company
may enter upon the premises of Customer at any reasonable
time and replace its regulating equipment and cylinders and
the gas content thereof. Company may enter upon the prem-
ises of Customer at any time and remove any cylinder which
has been disconnected by Customer and is not in use. Credit
on unused gas in such a cylinder will be allowed only for the
purchase price of the gas, less rehandling and reselling costs.
7. The performance of all agreements made herein by Com-
pany is subject to strikes, riots, war, fire, acts of God, acci-
dents, Governmental orders or regulations, curtailment of,
or failure in obtaining sufficient electrical power, lack of
transportation facilities, inability to obtain sufficient or suit-
able raw materials or supplies, conditions at the Installation
Address and other similar or different contingencies beyond
the reasonable control of Company.
8. Any notice provided to be given to Customer hereunder
,hall be deemed to be properly given when mailed to Cus-
tomer at the mail address specified on the face hereof.
9. This agreement contains all agreements, warranties and
representations between the parties whether oral or written
and supersedes all prior agreements with respect to the sub-
ject matter hereof. All implied warranties by Company are
expressly excluded. This agreement is not assignable by
Customer without Company's written consent.

By now you know some of the pleasure and satisfaction of a home
equipped with "Pyrofax" gaso We think: "Pyrofax" gas is the best
LP gas there is, and we're sure you'll have many years of pleasure
and satisfaction from using ito Here are a few operating and pre-
cautionary instructionso Please read them carefully and keep them
handy at all timeso
"Pyrofax" gas cylinders are made of the finest quality heavy steel,
carefully engineered and manufactured to surpass the requirements
of the Interstate Commerce Commissiono They are equipped with de~
vices designed to release gas if they are subjected to temperatures
of 1650F or high6lro Keep fire and excessive heat away from cylinders
and regulating equipmento
"Pyrofa.xf! gas, the cylinders and the regulating equipment all meet
all of the requirements of the National Fire Protection Association.
All work of con~~cting and dis~nnect.ing cylinders must be done
only by y~ur npyrof~'~, Gas Distributor or his trained employeeso
CAUTION
If gas flow has been interrupted, shut off burners
and pilots on all appliances before gas is turned on
at the cylindero
Do not use a flame in looking or testing for leaks as
"Pyrofax" gas is flammable and heavier than airo
Make sure that all gas-burning appliances you use are
suitable for use with propane eLP) gaso Do not use

familiarize yourself with this odor becavse it may differ from the
odor of other fuel gas previously used by youo In the event of
gas leakage close all cylinder valves immediately and do not turn

Thoroughly ventilate any oven, room, or other enclosed space into
which gas has accidentally escaped ~ keeping flames, sparks or
other sources of ignition away from the area until you are sure
it is thoroughly aired outo
CYLINDER CHANGE-OVER INSTRUCTIONS
Ao MANUALLY OPERATED EQUIPMENT
10 Turn off all burners and pilots on all appliances
immediately after cylinder in use becomes exhausted.
20 If manifold is equipped with valves close the
manifold valve which is nearest to exhausted cylinder,
open the other manifold valve and see that valve on
filled cylinder is openo
3. If manifold is not equipped with valves simplY
close valve on empty cylinder and open valve on filled
cylinder" To open cylinder valve~ Turn handwheel or
"T" handle wrench to the left (counter-clockwise).
To close~ Turn to the right (clockwise)o Be sure
to turn the handwheel or "T" wrench all the way when
closing or opening valve"

Bo AUTOMATIC EQUIPMENT
When in use, all cylinder valves should be open.
When indicator shows "Reservati or red, order
"Pyrofax" gas immediately - reserve cylinder is
then in use. You should not adjust, reset or change
equipment in any wayo
