AMENDMENT TO CONTRACT
THIS AMENDMENT to the original contract dated October 1, 1993 by
and between the City of Clearwater, a Florida municipal
corporation, having its principal office at 112 South Osceola
Avenue, Clearwater, Florida, hereinafter referred to as the "City"
and Pinellas County Headstart, a not for profit corporation, having
its principal office at 12351 134th Avenue North, Largo, Florida,
hereinafter referred to as the "Provider" in the arn,ount of Eight
Thousand Dollars ($8,000.00) is hereby being amended.
This agreement to amend the original contract is entered into this
day of , 1995 with an effective date of this
agreement being the 1st day of October, 1994.
WITNESSETH
WHEREAS, the City has entered into an agreement with the U. s~
Department of Housing and Urban Development for the purpose of
conducting a Housing and Community Development Program with federal
financial assistance under Title I of the Housing and Community
Development Act of 1974, as amended; and the Cranston-Gonzalez
National Affordable Housing Act of 1990; and
WHEREAS, the City has determined through its Year Eighteen
Final Statement of Objectives and Projected Use of Funds, which was
adopted by City of Clearwater on July 15, 1993, the necessity for
the Provider to purchase and install a modular classroom for the
Headstart Program at 701 N. Missouri Avenue, Clearwater; and
WHEREAS, the City desires to engage the Provider to render
certain services in connection therewith:
WHEREAS, the City has determined that the Provider furnished
sufficient documentation to establish a hardship for the Provider's
inability to complete this project under the terms of the original
contract; and
WHEREAS, the Provider maintains that an additional 10 months
is needed to complete the project; and
WHEREAS, the City hereby grants an additional 10 months to the
Provider to complete the project; and
NOW THEREFORE, IT IS HEREBY AGREED that the Provider may use the FY
1993-94 Community Development Block Grant Funds during the 1994-95
Fiscal Year, from October 1, 1994 until July 31, 1995. The Provider
agrees to implement this project in accordance with the Scope of
Services (Section I) of the original contract and abide by all of
the terms and conditions of the original contract, with the
exception of the term of the original contract. The term of the
contract shall be in accordance with the terms identified above.
